default
Posted on Mon, 2 Feb 2015
Question: for about a month I have been waking up once a week in the middle of the night with pain behind my right eye and a headache above my eye. Last night I woke up and had the pain but a real severe headache, one that I have never experienced, felt nauseas, it felt like my head was being squeezed between a vice, and it was on the right side of my head as well. I have pain in the cheeks but no nasal congestion. what is it?

Brief Answer:
Headache- Tension vs. Migraine- Should see headaches specialist

Detailed Answer:
Good evening sir. I am Dr. Saghafi from the XXXXXXX Ohio region of the world and am an adult neurologist as well as a headache specialist.

Your symptoms most remind me of someone suffering either from tension type headaches or possibly migraine headaches. Some of your symptoms are consistent with one type of headache while other symptoms you relate are consistent with the other type.

Since the problem has been around "ONLY" for 1 month we cannot say that is chronic yet..."minimum 3 months of time" or 5 episodes (you've had roughly 4 according to what you say) but those would be the 2 most likely types of headaches in my opinion. Of course, what the cause is and how best to treat would be very good questions to answer as well.....however, based upon limited information you've provided and the fact, that we don't even really have the diagnosis of certainty yet you can see how it would be difficult to know what the best intervention would be.

What I always recommend to my patients in clinic is that they keep track of their headaches on a special diary designed to track these types of events. I educate them in what to look for and how to fill out each of the parameters then, we analyze the log 3-4 weeks to look for the patterns which will bring together the most important characteristics of the headaches they have but hopefully, allow us to come up with an actual diagnosis which will then, point us in a direction of intervention and treatment.

BTW, there are apps on the iphone for keeping track of headaches. I haven't found one yet that, in my opinion, works as well for the information I'm interested in having as the diary that I pass out to my patients.

My recommendation to you is that you seek out a headache specialist in your area. Typically primary care physicians will not have this sort of tool readily available nor will have be as capable of interpreting the results in order to identify the most likely headache and suggest the best intervention compared to the specialist.

I wish you were in my area of the world because then, the solution would be much simpler. Of course, if you had any thought of coming to visit our Lovely Town of XXXXXXX which is RESTATING ITSELF on the map as one the Best Locations in the Nation to live and work then, I would extend the invitation to come and see me in my office....at any rate....

Thank you for the chance to help out. Happy New Year to you and Good luck!

If this information has been helpful would you do me the favor of some written feedback and a STAR RATING of the answer? STAR RATINGS are very helpful to the doctors because folks pay a lot of attention to a doctor's rating by his patients when deciding to ask a specific question or to return to him/her for service.

Should you have no further questions or comments to place- CLOSING THE QUERY would be as greatly appreciated as well.

And of course, in the future I'm happy to answer more items for you personally by going to the following website and writing to me:
