What Causes Pain And Pressure In The Groin Area?

default
Posted on Thu, 3 Jul 2014
Question: I have been dealing with pain and pressure in my groin area for 5 weeks now. I have been to 4 doctor's offices and no one has taken a blood test. All they have done is look me over and take urine samples and then tell me there were no bacteria present in the cultures.

I did get an ultrasound of my inguinal area and it showed that my lymph nodes are swollen. I can feel the lymph nodes, and they are small but still abnormal size and they have a burning sensation. I have had pain in my bladder area, but I think the infection has spread into my tissues.

I have made an appointment with an immunologist, internal medicine doctor for monday. I was told they most certainly will do a blood test.

I'm tired of waiting for another doctor's appointment that I have to wait several days to go in and be seen.

Do I need to go to the emergency room? I feel the pressure and discomfort has spread to my upper abdomen underneath my ribs. I'm really worried. Will they do anything other than a cursory exam and then tell me to go home? Do you think this could be some kind of internal staph infection? Do they have to biopsy a lymph node?

Hello Dr. Thakker,

I have no discharge, no ulcers, no fever.

I have uploaded the urinary and USG reports.

The pain is about intensity 3. My bladder area has stopped hurting like it was a few weeks ago. The tissues just above and on either side of my penis have a burning sensation if I press and massage the area.

There is constant pressure in my abdominal area and the lymph nodes in my inguinal area have a burning sensation. I feel the discomfort and pressure has spread from my lower to my upper abdomen below the ribs.

The pressure and discomfort is constant, it does not matter if my bladder is full or empty.

I am married with the same partner of 10 years.

No diabetes and no kidney stones history.

I think I can make it until Monday for my appointment. I just want to find out what the cause of this is.

Thank you,
XXXX

Brief Answer:
epididymo orchitis, prostatis, systemic causes

Detailed Answer:
Dear XXXXXXX

I have seen your urine and ultrasound reports. The most likely cause of your pain is Epididymo orchitis. This is due to infection by gram negative organisms like E.Coli or chlamydia. They cause enlarged lymph nodes as well.

Another possibility is prostatis.

We also need to consider systemic causes of inguinal lymphadenopathy like lymphoma, TB, HIV, Sarcoid.

I woud recommend you the following:

Gram staining of secretion after prostatic massage, urine culture.

HIV ELISA.

If there are negative, we can perform a FNAC of the lymph node to rule out a lymphoma.

In the mean time you can take a quinolone like ciprofloxacin for treatment.

I hope this helps you out. If you have any more questions, please do not hesitate to write back to me.

Yours sincerely,
Dr Thakker.
