Brief Answer:
Diabetician followed by a cardiologist

Detailed Answer:
Hi
I am Dr Mittal
There is possibility that your sugars are too well controlled.
Therefore during the night, the sugars may be dropping below the needed levels.
You will therefore need to meet your diabetician to review the doses of the medicines.

Also, in diabetics, the heart attacks may happen without chest pain. They have symptoms like sweating breathlessness fatigue etc. And therefore, I would also suggest a EKG and echocardiography.

I hope that helps.
Please feel free to contact me for more details.
Best of luck
