Question: Hi Doctor I am 57 year old Female , I discovered higher sugar level than normal couple of months back and have been having around 120-130 (before meal) , 165 - 180 (2 hours after meal). I tried to bring these level down with around 4 Km walk twice a day. Then I tried ayurvedic medicine with which it came down to 111 and 140 but medicine didnt suit and led to stomach issues , gas etc..Now i shifted to normal allopathic doctor and i was advised Gluconorm SR recently.Problem is it is not suiting me. feel Nausea and weakness and trembling. Also feel lot of gas due to this. Can you please advise if there is alternate salt medicine which can suit and then i will re consult my doctor. My medical history - Gall bladder removed 20 years back due to stone , since then general gas and acidity observed and take antacid for that (Zintaec 150). Also dont have very good digestion.BP used to be little higher and hence started medicine tenoric 50 and now olmesar ch 40 after observing sugar (as advised by doc) With this BP has been in control and fine. Also have Thyroid for which have been taking Thronorm 50.

Hi,Madam,
I have carefully read your query and understand your concern.

Gluconorm SR is associated with few significant gastrointestinal disturbances.

This is overlapping on the already existing gastrointestinal disturbances post cholecystectomy.

Instead of Gluconorm,new antidiabetic drugs like Gliptans ex Sitagliptan can be relatively free of side effects mentioned above.

To combat these side effects ,i recommend Tab Esmaprazole 20 instead of Zinetac 150mg.The former drug is quite effective in controlling the side effects.

Note :These drugs have to be taken only under strict medical supervision.

Thanks Doctor , have few more queries. Zinetac I used to have earlier till the time sugar wasnt discovered. Since Sugar was discovered I started REPEPSA but didnt find much effective. Can I use Pantocid DSR instead as I have used that in past sometimes and found it effective or shall i go for Esmaprazole 20 only and is more recommended?
Whenever I take any antibiotic for any sickness I make sure i take such antacids along as mentioned above along else my stomach always suffers , its sensitive and hence I just try to avoid to take much medicines and antibiotics


Also one more input from medical history is once doctor said that some outer intestine burning like ulcer (though it wasnt inside like ulcer) has happened hence you must avoid pain killer. There is no such problem now though and it was 7-8 years back
Coming back to sugar medicine you mentioned gliptin , is JALRA 50 (vildagliptin) tablet having same salt and can be used? Or if its different shall go for Sitagliptin as mentioned by you (certainly all these changes after consulting my local doctor as well).

Another point just for information as i mentioned in my initial query I have taken ayurvedic medicine first called madhunashini (Ramdevs patanjli tablet) - will you be able to comment on that as well? Since i taken this for 45 days and now recently only started gluconorm sr 4 days back. So in case you recommend madhunashini may try that also for longer if it has less side effects

Hi Madam,

Esmoprazole is better and long acting than pantoprazole.

I strongly recommend you to have an upper git endoscopy with H.pylori study in view of significant past history for gastric ulcer. Its mandate that you rule this out since they can exacerbate your present symptoms. In any such case pain killers are to be avoided.

Vildagliptan is more effective than Sitagliptan in reducing the mean average blood glucose, average glucose excursions, post supper and post breakfast hyperglycemia. Vildagliptan twice a day dose of 50 mg each is effective.

Ayurvedic drugs are not effective in controlling diabetes. You may not take them.
