What Causes Miscarriage During Second Month Of Pregnancy?

Question: hi. About 3. 5 years ago, wife had a miscarriage, and a failed ivf 5 month ago. At the moment the HPT has come positive and beta hcg was 365.9, post 48 hours this was 414 and 5 hours post 414 reading the same was 433. The increase is not 60%, she did her USG last week itself within hours of the BHCG radiologist said neither intra/extra ueterine gestational sac is seen at the moment, and said this means this has been flushed out or too early. Given this background based on non visibility of extra uterine gestational sac , can one confirm there is no ectopic preganancy. LMP was 24.2.16 and BHC done on 8.4.16
Is there a chance of normal pregnancy? Can we based on this reading confirm that there is no ectopic pregnancy at this stage?

1) According to last period date, current gestational age is more than 6 weeks. At this stage, gestational sac, yolk sac and fetal pole should be visible.

2) Beta-hCG is NOT raised in NORMAL pattern and its value is almost constant even after 48 hrs. Therefore, it neither looks like healthy pregnancy nor the ectopic pregnancy. It is mostly suggestive of early miscarriage or flushed out.

3) I suggest to repeat Trans Vaginal Ultrasound scan and beta-hCG test in this week to confirm the status of pregnancy. Review with test reports.

Just one last question is dr has suggested that the cycle is irregular and there is a chance that the late ovulation may have caused this?
She has been prescribed duphaston, susten, folvite and ecospirin.

If her cycle was irregular, then late ovulation or conception should be ruled out. In that case, she has quite chance of continuing this pregnancy.

You should repeat the TVS scan and beta-hCG test for confirmation of the status of pregnancy. She must continue above said medicine. Maintain genital hygiene properly.

Hi. Dr. We have done a TVS on Saturday as discussed in my earlier mail.
The USG has given the following an Intra uteritne gestational Sac.
The Gestational Sac diameter is 10.2 mm and he has said this corresponds to 4 gestational age.
He has said there is no evidence of fetal pole or heart beat. Is this worrisome given our context?
He has asked to come back after 2 weeks for Sono.
The Gynac doctor has asked to continue the following medicine for 2 more weeks, are these safe?
Duphaston-1-0-1
Susten 300 mg-1-1-1
Folvite-0-1-0
Ecospirin-0-0-1
