What Causes Mid Cycle Menstrual Bleeding After Delivery?

Question: I am 13 months post partum, and stopped nursing about 5 months ago.

Since then I have started menstruating again, on a fairly regular basis. For the last 4 months, I have had mid-cycle bleeding, which I have never had before. The blood is mostly brown or pink, but occasionally clotted and somewhat stringy. It is more than spotting, but certainly less than a full period, and tends to last for 4-5 days. I am also getting lower back cramping, but no other pain. My periods are otherwise normal. I am not on birth control (I have had a tubal ligation), I am hypothyroid, but on meds, and my TSH is normal. I have had weight loss, but it's steady. I had an internal exam a few weeks back and was diagnosed with BV, which I was treated for, but the symptoms there were very different, and it seems the mid-cycle bleeding was just coincidental. I'm not sure if this is a hormonal imbalance? Polyps? Any thoughts on the protocol here?

Irregular mid cycle bleeding with abnormal brown to pink discharge may be due to hormonal imbalance, mental stress, Pelvic Inflammatory Disease, endometrial or cervical polyp, over weight, thyroid disturbance etc.

Do you undergo any pelvic ultrasound scan or hormonal profile recently? If yes, kindly attach them.

No ultrasound yet and haven't seen an endo for hormone profile. My dr seemed to think they first year postpartum can be a little off. Thyroid is normal (2.5 TSH). I am losing weight, and at a heavier weight, never had issues, but I am almost at a normal weight. Not PID or STD.

I suggest you to undergo one pelvic ultrasound scan and hormone profile rather than Serum TSH. It would be worthwhile to make a complete treatment.

It is the fact that most cases of irregular bleeding in postpartum period is due to hormonal imbalance. However, we need to rule out other pelvic pathology (if any) as mentioned earlier and confirm the diagnosis.

You may require hormonal treatment to arrest irregular bleeding. Take healthy balanced diet with IRON supplements and maintain genital hygiene properly.
