Question: Heart failure - stage 4. Medications started .Recent bloods showed poor liver/ kidney function. Wht can this mean ?

He has a icd machine . But duno how collapsed . Plus liver and kidney readings and abnormal what could be the cause?

Thanks for asking query again,
As per your query detoriating liver and renal function are suggestive of poor perfusion(blood supply) to these organs.
As EF is just 15% blood supply is severely hampered leading to detoriation of renal function.
Intervention should be done immediately, Medical management is not helping your patient.
Patient is prone to sudden cardiac death.

Could readings with liver and kidney be out of range due to side effects of the medications ? Or dehydration from restricted fluids?

Most likely not.
Deranged liver and renal parameters at the same time in patient with 15% ef is suggestive of cardiac pathology.
Frankly speaking don't google too much.
Try to understand your patient is detoriating.
Don't go for rare scenarios first rule out common causes.
Its heart in this case.

U are totally right. But thing is he was in hosp 1 week after the stage 4 diagnosis and all bloods were ok. As soon as mediations started , showed kidney n liver out of range. Could
He be dehydrated ?

Hi,
Please message to me the lists of medicines he is on.
Some medicines like statins are hepatotoxic while medicines like ramipril should not be given in patients with renal problems and deranged KFT.

Keeping hi dehydrated is our goal.Fluid intake(total soup,curry and any thing which has liquid) hould not be more then one litre in a day otherwise CHF will precitate.
Vert vert strict fluid restriction is to be followed.

Yes of course why not they can. Especially statins, Diuertic's and ace inhibitors
Almost all medicines are metabolised by liver and excreted by kidney.

Only possible explaination to your question is through example-
Suppose Two scenario's are there.

1) Your house would be robbed by thieves,But they promise that if you give them your oraments without resisting they wont touch any one or harm your family.

2)Your house will be attacked by looters.They have history of looting as well as causing physical harm to person and family.

If you have to make a choose to choose one out of two,Most likely you will choose the first one.

In the same way in a person with grade 4 CCF symtoms with EF 15%,Medicines may cause some side effect but if patient would die after 1 year on medicines without them he could crash in few days or hours.
Prons of therapy-Chances of life increased.
Cons of therapy-Yes off course no side effect of medicine but he would have cardiac arrest in two weeks if EF is 15%,
