What Causes Jagged Lines In The Field Of Vision?

Question: I am a teacher. At dismissal time today I had a very strange thing happen to my vision. On my left side it looked like I was looking through broken glass. It was very strange. It lasted about 10 minutes. I was not light headed or shaky. I was just looking through a shiny hazy peripheral view. I have a diagnosis of Graves Disease. I am on 5 mg of methimazole daily. I have had an MRI to test for MS. It was normal. I have a low positive ANA of 1:132. I have an ESR of 25. It scared me. I just wanted to know if it is something to be concerned about.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

The symptoms you are describing appear to be of a scotoma that improved after sometime.Common causes can be-

1)High blood pressure
2)Migraine
3)Mini stroke or transient ischemic attack
4)Demyelination

Normal MRI has essentially ruled out demyelination and stroke.You need to be screened for high blood pressure and managed,if found so.

Further,you should get funduscopic evaluation done by an ophthalmologist.Also,Visual evoked potential(VEP) should be done,which may contribute towards the diagnosis.

Graves disease should not be having a direct relation with scotoma. ESR as 25 should not be a cause of concern.

Though,positive ANA can be an indicator of vasculitis.However,Longitudinal observation is needed and if such an episode occurs again,evaluation for vasculitis should be done as well,as a part of the diagnostic algorithm.
