Question: i need to consult a rheumotologist for lupus nephritis
, my wife is in full remission as per tests since 4 years now after going through a chemo therapy. she as also hashimoto but her TSH seems under control. she is taking thyroxine
when her bp fluctuates reaching 160/100. we raise the doses until she goes back to normal 120/80
but still this happens every few months, reasons we dont know

I have gone through her reports and her Kidney functions still seem to be high...Maybe a bit of scarring has been left earlier on the kidneys

As for the blood pressure, I would like to know what medications is she taking??

2. Are the medicines being taken on continous basis or on intermittent??

3. Did she have blood pressure medicine before as well???

4. Is she taking Hydroxychloroquine for her Lupus??

her kidney function is much better than before , she had severe lupus nephritis in 2013 , of course there scarring but she is in full remission
her bp Med
amlor 2.5 mg we raise to 5 mg in case of bp
concur 2 mg or 5 mg
diovan 80 mg
most of time her bp is 120 / 80
but from time to time is fluctuates and we don't know why

she is taking also plaquinel 2 tabs per day

Sorry for the late reply..Had a long duty yesterday

Well, as for the blood pressure variation, some patients do suffer from Labile Hypertension, that is they will have intermittent shooting up of the blood pressure...But just to be sure there are no pathological cause for it, I will advise a few blood tests:

1. Get a repeat REnal Doppler done..They may have done the ultrasound but one needs to look at the blood flow as well..Kidneys are a common cause of BP

2. 24 hour urine for VanylMandelic Acid and Metanephrines

3. Also get a 24 hour BP monitoring done...it will record her BP over 24 hours and we will know if she keeps getting these intermittent spikes every now and then...So we can adjust her medicines accordingly...

As for Plaquniel, I feel she can cut down the dose to 1 tablet( 200 mg daily ) as well since she has been on it for a while

Well, these are the causes which are reversible...That is why we should look into these to see why there is labile hypertension... Most of the times, one can find the cause and hence we need to cope up with whatever the situation comes...24 hour BP monitoring will give us an exact clue at what time the BP shoots up and during what activity, so we can time the medicines accordingly....

I am sorry if waiting is such a problem for you cause I do have my Hospital services to attend as well, and I try to reply as soon as I can...
