Question: Hi Dr. thank you for your help. For a little over a month now, beginning with either a splinter or possibly what I thought, though now don't, believe was a deep pimple on my buttocks has now grown firmer and what puss, etc. that is discharged smelled initially rotten now has taken on a bleach odor. Abscess? Until recently is didn't hurt, now though, between its size and firmness, well, ow.

Brief Answer:
probably a perianal abscess

Detailed Answer:
Hello.
Welcome to HCM.
This is Dr. Swarnava Dattagupta answering your query.

I understand your concern.
From your description, it looks like a perianal abscess. Initially it can start as a small lesion but as you have mentioned now its size has increased, symptoms have also occurred. There is also presence of bad odoured pus.
If such a patient came to me i would ask him to undergo a course of antibiotic like coamoxyclav for 5 days. For pain, mild painkillers would be suggested. If it doesnot help, incision and drainage of the abscess needs to be done. Cleanliness and hygiene of the abscess site needs to be maintained too.

If you can send a picture of the abscess it would be more helpful.
Hope this was useful.
Consult me for further queries.
Regards.

Thank you Dr. I'm having trouble with my camera/phone but your description sounds firm and your suggestions of immediate value. As far as how it looks, it's raised with a solid mass-like feel within and reddening with a slight purple deepest towards the center, very firm and on earlier attempts at squeezing hoping to pop or extrude any foreign matter, squirted a thin but milky with some blood puss. At present is is a dull but ongoing slightly burning pain.

Brief Answer:
Start antibiotics; visit a doctor

Detailed Answer:
Hello again.
From your description, it seems like a chronic abscess. Probably, the infection has persisted since the beginning till now. However, there is not much pus now, nor there is excruciating pain. This goes in favour of chronic abscess. I would suggest antibiotics and visit to a doctor, preferably a general surgeon.

Hope this helps.
Regards.
