Question: Twenty eight year old female postpartum 3mo two weeks excessive hair loss facial swelling pain behind eyes feels hot shakes feet tingling anxiety metalic taste mouth went er Baylor XXXXXXX Tx yesterday they puzzled rx for allergic reaction possibly I am thinking thyroid pituitary problem I am mom family history thyroid disease father ablated twice mom hx forty thyroid nodules none hot I feel she needs go back in er sat nothing open as far dr offices endocrinologist work up

Good day.
Noted your case history.

I agree with you that the symptoms could be due to thyroid dysfuction. ( however, many of these symptoms do occur after pregnancy due to hormonal changes). It may be worth checking a Free t4 and TSH. This is especially important beacuse of positive family history. It may be worthwhile to get a CBC, ESR tests as well. There are all blood tests. Please let me know the reports.

Regarding pituitary dysfunction, you need to get a blood pressure check up lying and standing. After the delivery, how is the milk secretion? Is it less or normal. Did she have a prolonged labour or excess bleeding during delivery ?

Thank you Doctor for the information which I will pass on to my daughter. She had a normal delivery although six weeks premature baby boy is fine growing quickly and had no complications or excessive blood loss. Will pass on bloodwork results when I get them. Encouraging her to go in despite not having medical insurance. Regards XXXX

I would strongly recommend a Free T4 and TSH, CBC and ESR tests. Your generaly practioner can arrange these tests and they are not expensive. If she has a normal blood pressure and thyroid function tests and if she is producing milk in normal quantities, a detailed pituitary evaluation may not be necessarily needed.

However, your general practioner ( GP) may be able to pick up clinical clues during pituitary dysfunction. It may be importnat to do a clinical test for visual acuity and visual field tests. Your GP can do these test in his/her office itself. If he is not suspecting a pituitary dysfunction clinically, then we shouldn't proceed in gettng all expensive pituitary blood works.

She has agreed to go in tomorrow indicates her blood pressure and pulse are high for her will take it while lying down as well she is a tiny gal tall thin breast milk sparse at first but now has increased

Oh good. I was concerned of a low BP especially while standing ( on changing posture from lying to standing)

If BP is not low, and if breast milk is good, major pituitary problems are unlikely.
