What Causes Excessive Day Time Sleepiness?

Question: HI
I am suffering from excessive day time sleppiness.I dont have snoring problem. and I feel fresh after waking up in the morning. But I feel sleepy from 9 AM till evening . I am taking modafinil 100 mg once in the morning and another in the afternoon. I also feel that when I am working in the XXXXXXX area, I feel less sleepy and I can be able to control my sleep without taking XXXXXXX medicine or coffee. Can this be a case of lack vitamin D. Can I take multi vitamins in addition to modafinil.Should I consult a neurologist or any other department

Ans- This is happening every day.

2. Are you facing any work related or personal stress?
Ans-Of course, there is work related stress. But it is mostly due to low performance because of I feel sleepy. So I work slower than any other person. I feel shy to tell my colleagues. I work by taking lots of green tea, coffee and other stimulants . It also consumes 2-3 hours per day. So it reduce my performance and accuracy.
Some times I work on Saturday and Sunday.

3. What is your occupation?
Ans-I am working as a legal manager in a General Insurance Company in XXXXXXX My work is to read , research and provide opinion, involves responsibility . It does not involve any outdoor activity.
4. Are you married?
Ans- Yes . I am married for more than 2 years and don’t have planned for any issue . I am staying with my wife. My parents and brother also visit to us frequently. I don’t have any ill feeling in my family or any other relation is not affecting my life.

5. What is you sleep schedule during night and your routine activities.
Ans- I used to go to bed around 11.30 to 12 at night and get up around 6.45 to 7.15 in the morning. I never like to take nap in weekdays nor in weekends. My sleep pattern is deep and don’t have snoring problem. My sleep is so deep that once I sleep I don’t get up if any phone rings/in sound of TV or crackers. I used to sleep by 5 -10 minutes of closing my eyes and sometimes less than one minute.

6. When I get up,I don’t feel active. I have to take tea or do kapalbhati(yoga) on the bed to make me feel active. Then I do other work.

7. By taking 2 modafinil 100 mg at a time, I felt restless, trembling in my leg and uneasiness. If I don’t take 2 tablets I don’t feel active.

Thank you for the information.

After going through your history, both obvious psychological and physiological stress can be ruled out. The condition you are currently suffering from is called "Narcolepsy".

Before I move on to advise you on the therapy, here are few very important things you should always remember and take care off.
1. Modafinil may decrease sleepiness, but it is not a complete remedy and does not cure sleep disorders. 

2. Take modafinil at the same time every day. Do not change the time of day or dose of modafinil without talking to your doctor.

3. Narcolepsy is a complex brain condition precipitated by stress be it physical, physiological, psychological. There is no established complete cure for narcolepsy. Many people have got complete cure of it. For everyone the same treatment does not hold good. Few factors work for some resulting in complete cure. Lots of patience and spending time with self for that inner realization to understand what exactly is required to get your mind at peace would be required from your end.

Here are few advise:

1. First and foremost make sure you have good night sleep every night undisturbed

2. Forget all your worries and in differences with your family and friends before going to bed

3. Ask yourself if you are happy with your current job. This is where you spend your day time and you tend to end up sleepy.

4. If you are not happy with work or work place, immediately start searching for new job and resign from this place.

5. If you are happy with both work and work place, then try to spice it up. Make your seat and desk colourful and bright with accessories and photographs as permitted by your work place. Make good friends at job and try to spend coffee time with them. Healthy gosspis too are welcome;) You will start loving going to office and would like to keep yourself awake to meet those good friends and for all the lovely chat you guys have.

6. Spend time with yourself and find out what is that one thing gives that adrenaline rush and you would want to do it again and again. Repeat that work during day time for next 21 days and ensure you don't feel sleepy while doing that task you love. I can bet you that you will not feel sleepy on 22nd day because an habit is already formed.

6. Listen to good music especially flute. It calms mind and gives XXXXXXX

7. Read motivational novels and books.

8. Go back to college days and remember the goals you had set for yourself. Just analyse where have your friends reached today. Re set your goals and these goals will not allow you go sleep till you accomplish them.

8. Take yourself little seriously and start doing good things for yourself like spa massage, partying with friends and family, meeting old friends, pursuing old hobbies etc..

Please follow above advise and you will notice great improvement in self. Of course there are plenty other things to be discussed. We shall discuss and reveal it slowly.

Hello Madam

Please guide me with my further queries.

1) Food habits- what food habits will make me feel active especially help me in control of excessive sleep.

2) can yoga or aerobics etc helpful for me
3) can change of climate will affect me
4) cjange of city by moving to hotter or cooler area will help me

A small picnic or outgoing especially in places of your interest will rejuvenate you.
Change work place if you are not happy.

Go out for some picnic.
