What Causes Episodes Of Chest Pain, Arm Aches And High BP?

Question: Yes I have been told several conditions that I have. TOS,RA that with treatment is now down from severe to moderate. But I have these episodes where I have chest pains neck and left arm aches with numbness and blood pressure high and will change up to 40 points lowering when standing higher while setting. Hart rate that is 180 to upper 190s and loss of vision this last time. I can make this happen with certain activities within 5 min. But can pass stress test just fine. What should I do or see or could this be? Lost and need help I am 49 and my life has changed suddenly in the last 4 years. Please give direction thank you.

Good day,
Noted your concern. Sorry to hear about your symptoms. I suspect atleast some of your symptoms could be related to REMICADE side effects. However, we need to rule out thyroid diosrders and rare other hormonal conditions ( especially hyperthyroidism and pheochromocytoma) which maybe causing your symptoms.

May i suggest,
1) ECG and holter monitoring.
2) Thyroid blood profile
3) 24 hour urine Metanephrines and nor metanephrines ( some of your medicines may need to be modified for couple of weeks before doing this).

Noted that, stress test was okay before.

Having saiid this, anxiety plays a big role in such symptoms. Do you consider yourself being anxious/depressed ?

Discuss these point with your primary care doctor.

Yes they have done the halter monitoring that is when the Toporal was prescribed twice daily and the urine test was perfect also sonogram and ct with contrast. Thyroid test and other metabolic test. All came back good. Other than a 6 mm pulmonarynodule in the terminal branch pulmonary artery aneurysm to have periodic surveillance. This was caught on the ct renal study.The problem was prior to the Remicade began. I dont smoke or drink alcohol and mainly drink water around 8 to 10 bottles a day an maybe one soft drink. But have be dehydrated and renal failure. That was when they found I had sever RA. I have a cardiologist and vascular and kidney doctor as well my RA doctor. They are puzzled and suggested a endocrinologist might have an answer. I was taking a different medication at first for the RA that wasn't work and made my testosterone level to go to nothing but it rebounded once stopping. What other avenue or testing could you recommend. The vascular specialists sad some of the pain and other symptoms could be the TOS but that the blood pressure and high heart rate was not?

Dont worry about that pulmonary nodule. But need follow up.

It is not clear what kind of urine test was done in you.

You need 24 hour Urine Metanephrines and metanephrines
Besides, you may also need to do 24 hour urinary HIAA levels, a blood Chromogranin-A levels, etc.

I feel yourself need an endocrinology evaluation. It is best to examine you and then to decide the tests .

Some tests other than CT may be required based on the above Urine and blood tests ( only if the urine/blood tests are suspicious).

By the way, you didnt answer my last question.Do you consider yourself being anxious/depressed ?

No not really i feel aggravated that i cant do the things and have the energy that i did have. But nothing that if i could get the health isues corrected would not resolve. I cant say that it hasnt impacted life it has very much. So i have had to cut back and try to not push my self as hard. Which does cause some added stress and has made me not as out going as i was. But no more than i think anyone that has to deal with the problems that i am facing now would have. But when i get rushed when working it does cause some anxiety that i think is because for most of my life i havent ever had anyone that could out work me. I dont mean that to sound conceited just to give you an ideal of my work ethic. I feel blessed that i have had the ability to do more than most and enjoyed working hard. I am not sure what all was tested in the 24 hour urinary test it says Metanphrines/VMA/HIAA/Cortisol/Catecholamines WNL. On the XXXXXXX protocol showed no ekg changes Rare PVCs normal ETT then no AFIB or SVT Episodes of sinus tachycardia up to High HR 198bpm Average HR 86bpm Low HR 50bpm the Renal artery sono. Echocardiogram LV normal size EF 60% Mild DD Cardiac XXXXXXX normal Peripheral arterial testing reveald vasospasm in the right hand. So where to go?

Glad to know that the urine tests were normal.
Since all the said tests are normal, i would assume that TOS itself could be causing the chest/neck and left arm pain. When you get these pains, you tend to be anxious( panic state) which push up the blood pressure.

You may need to meet an endocrinologist in person to examine you to look for clues of any other hormone over secretion.
