What Causes Elevated Platelet Count And Free Thyroxine Level In Blood?

Question: I recently had a complete blood work-up done and while everything came back normal, except for too much thyroid hormone (am on Synthroid), my platelet count was higher than the normal range. Should I be concerned? The PA who went over the lab results with me wasn't concerned, but would like to know more. She said platelet count can vary even day to day.

Hi,

Welcome to HCM. I have gone through your question and understand your concerns.

Please let me know the platelet count value and thyroid hormone test report.

1. Free t4 (thyroid hormone) can be slightly elevated when you are on synthroid. We adjust Synthroid dose based on TSH value. So if your TSH is normal, no need to worry.

2. The electronic blood count machines normally sometimes show high platelet count. You have to confirm it by peripheral smear examination (manually seeing the platelets). If it is also showing high counts you need to be evaluated.

Infections, vitamin &iron deficiencies, trauma, exercise etc all may increase platelet count. So if the count is only moderately elevated, you can repeat the test after 2 weeks before going for detailed evaluation.

I had a total thyroidectomy 20+ yrs. ago for thyroid cancer, so am on Synthroid for that reason.
So I should check the platelet count in 2 weeks? Thanks for your time.

Hi,

In thyroid cancer we slightly over dose thyroxine to keep the TSH on lower side of normal. That explain your high thyroid hormones. Shall review with repeat platelet count.
