Question: Hi Doctor,
My wife recently got some small rashes on hand, below the wrist. I have attached the pics which shows these rashes, could you let us know if there is any thing to be concern about?

Hello. Thank you for writing to us.

I have taken note of your query and I have also reviewed the Images.

I can see dry, red, rough patches of skin on the dorsum of hand.

I suggest a possibility of polymorphic light eruption (PMLE). PMLE can presents as itchy, red, raised, papules or plaques of eczematous skin involvement.
UV A and UV B part of the electromagnetic spectrum are implicated in PMLE.

She should protect her skin from sun exposure.
In addition she may use a broad spectrum sunscreen, throughout the day, on all exposed skin and reapply every 2-3 hours.

She may also use a potent topical steroid e.g clobetasol propionate 0.05% cream once daily at night to help resolve the patches.

An OTC oral antihistamine e.g cetrizine 10 mg once daily will provide her symptomatic relief from itching.

Hi.

I will simplify it for you.

This eruption is due to sun exposure therefore she should protect from sun.

1. She may also use a sunscreen during day time.
Neutrogena ultrasheer dry touch sunblock (SPF 50+) is an OTC sunscreen and you can try to find at a chemist shop near you.
She may re-apply every 2-3 hours, through out the day (i.e 8 AM, 11 AM, 2 PM)

2. At night she may use a topical steroid cream e.g clobetasol propionate 0.05% cream Or betamethasone dipropionate 0.05% cream, over the affected area.
This cream would help resolve the rashes.

3. Once daily tab cetrizine 10 mg in evening would provide her symptomatic relief from itching. It is available OTC
