Question: Hello,
Im 22 years old, i suffer from asthma,chronic pelvic pain, chronic pain, fibromyalgia, depression and anxiety.

My questions are im in a flare up which i think is due to stress but im getting dizzy spells is this due to the higher pain ? I do suffer from vertigo but this feels different.
My next question is im due to gave my right submandibular salivary gland removed due to stones and damage im very scared and dont know if im making the right choice to have surgery nothing else theyve tried has worked?
Also i keep feeling very breathless and wheezy yet ehen i tell my gp he says my chest is lovely and clear but my reliever inhalor is working on these episodes ? Is it asthma related ? Im on seretide, ventolin, pregabalin 150mg, pregabalin 50mg, paracetamol, codiene, co-codamol, ranitidine and cyclizine, and cetirizine
I dont take codiene and co-codamol together but got thrm if i need tgem nor do i take paracetamol and co-codamol.

Read the history carefully. I will change the chronology of my answers.

Second question first- The submandibular gland. If it is confirmed by CT scan that there are multiple stones in the gland and is damaged, the only way is to get it removed. This is a very simple operation in the experienced hands. What are the other things they tried ?

The first question- Yes, the stress is the Mother of many problems you are suffering and can easily flare up the problems you have. The dizzy spells again can be a part of your symptom-complex. The real vertigo is a feeling of rotation and unable to control the fall or so. Otherwise a feeling of falling or blackouts is also taken as vertigo- it is not vertigo.

Inhaler can also give a sense of relief from psychological method. The bronchospasm of asthma can not be messed on auscultation by a stethoscope, yet it can be proved with pulmonary function test, usually done by a Pulmonologist. He will also examine you , investigate you, treat you, Make you fir for anesthesia and surgery and fitness given by him is very important.

Well, the medicines part is always evidence based, meaning is prescribed by the Doctor on clinical examination, please continue as per the guidance of your treating Doctor or a Specialist.

Hi they have tried prolonged course of antibiotics didnt work they also did a dcan showing damage to the gland
But im terrified im making rge wronge choice as been told i may end up facially paraysled which is frightening me hugely
Alsi im trying to refuce my stress but fibding it very hard to do my pain has flare up im feeling exhausted and very sicky im back to having to nap also having to use the codiene more im feeling breathless and generally frightened to be conpletely honest

Facial palsy does not occur in a normal course

Hi.
Oh, Who told you about facial paralysis ?
Never appears in this surgery at all. The facial nerve is high above and does not come in view at all. It is only the last twig that may appears and as this can be easily seen and tested on table by the operating surgeon , there is no chance of facial paralysis.
So just relax and forget about this aspect.

Since a long course of antibiotic has been tried and scan is showing damage , the submandibular gland has to be excised.

Why should you be stressed out so much; please tell me, I may be in a position to tell you the exact relaxation technique. Take minimum most medicines. Worries do not solve any problem. Facing the reality is the best solution. The stress is automatically reduced.

You and your surgery are going to be most successful.

Hi thanks for getting bk to me.
Im stressed out due to this operation also i believe i have a UTI which isnt helping as im in a lot of pain in my lower tummy and down below and it hurts a lot when i pass urine got sensation i need to wee when i dont or very little comes away also the wee is very strong smelling. This has only started this morning and gotten worse as day has progressed.the pain down below as well feels like a period cramp but a extreme one ! Also im bately sleeping at the moment

The simple test of Urine- routine, microscopy, culture and sensitivity along with ultrasonography will help to confirm a diagnosis and you may get started with antibiotics after you give the urine sample.
In may patients the anxiety can lead to frequent urination.

Consult your treating Doctor and get this done and get relaxed. You may request for medicines ( prescription medicines ) so that these problems too can be alleviated.

No one is perfect and everyone goes through such problems in one or the other forms. Discuss with your parents / family and friends. They would certainly help you to pass through such situations.

Hi, Your welcome,
My Ear, nose and throat surgeon told me regarding the facial paraysled as he said he has handed my care over to another surgeon on his team as it is a huge risk and one he personally doesn't want to have to deal with and feels his colleague would be better able to deal with it being a neck surgeon, also when I went for my first pre-operative assesment, they also told me there as well,
I am wondering as I have been told by a friend who says a dr told her to, to Take paracetamol then 15 minutes later take codiene then after a hr if pain is still bad take one paracetamol She said this works for her is this safe?
I also believe I am struggling with the stress at the moment, as as soon as I get better from something, something else is cropping up, which makes me wonder if I should carry on battling as it seems to be a never ending circled.

I do have one other question, Since I had my appendix removed laproscropically in 2008 possibly 2007 can't remeber year I suffer from very bad pain on the right hand side of my stomach, which is continuous I have seen the surgeon on two occasions who said he doesn't know what is going on, As I am always sick with the pain as well I have been taken back to A&E several times due to the pain and they can never find a answer, What could be causing it as, I am on medication to stop the vomiting but doesn't work, I have even gone down the avenue of being pregnant but all tests and scans came back negative Everyting I do now has to be planned down to the last detail due to being sick, I have countless times had to cancel plans due to this which is starting to get me down as well, Any idea's as to why?

Explaining the risks is a part of the pre-operative procedure and are abide by; this does not mean that you will develop that certain complication. And the risk of keeping a damaged submandibular gland is far more than this so-called risk. We have to weigh the expected benefits and the possible complications to decide. Here the benefits are far more.

Why would you follow a friend for the dosage and the way paracetamol and codeine be taken. This is safe as you are already taking this and follow whatever way suits you.

It is good at least you have accepted that you are struggling with stress. Consult your Doctors to get better medicines to tackle this. You alone can break the vicious cycle of getting better with something and the ext crop-up.

Noted the Appendectomy and the pain on the right side can be due to irritable bowel syndrome which is generated by your stress and anxiety. You can request for colonoscopy, just to rule out any other problem.

In spite of planning yourself, I would advise you to have a bit blind faith in Doctors and request them to help you. I am sure that the Doctors in contact with you are considerate, understand your problem and help you by appropriate medicines to tackle the stress.

I am sure you will cope with this well, you are young and should be strong.
