Question: I am a 62 year old male. I am noticing that my penis seems to be getting smaller. And now I am having difficulty in ejaculating. What should I do?

First of all, it is sad to know what you had been experiencing lately.

Based on the history you have shared, though shortening in length with age can be a normal phenomenon if over the recent part of the changes had been significantly quick, it needs to rule out other causes like essential vitamins and micronutrients deficiencies. They may improve with the replacement of vitamins like D3, B12, B6, B1 and folic acid along with vitamin E which improves sperms motility etc and micronutrients like zinc and calcium.

With age, the hormonal surges also vary and that may also contribute to decreased libido and decreased erection or ejaculation and that may need an Endocrinologist review for replacements and shouldn't be done without proper level assays and cautionary replacements as side effects need to be looked at before starting such therapies.

A Urologist review and ultrasound for prostate size may also be needed to rule out any mechanical obstruction to semen ejaculation.

Also using drugs like Sildenafil after discussing with the Urologist after a baseline cardiac work up (to rule out cardiac contraindications) may help. Wishing you a speedy recovery.
