What Causes Cramping,bloating,diarrhea,nausea And Appetite Loss?

Question: Hi, For as long as I can remember I have had stomach problems (17 years). I was diagnosed with acid reflux and IBS although never had any testing to confirm this. For the past week or so I have had cramping, gas, bloating, diarrhea, loose bowels, nausea, and lack of appetite. This happens to me occasionally and once I have bowel movement the cramping will usually go away but always leaves me feeling nauseated and unable to want to eat. I was told by my doctor about 5 years ago that this was related to IBS. I was wondering if there could be something else causing this or anything I can do to help these symptoms. Thank you.

Brief Answer: Change in dieatary habit, and some investigation Detailed Answer: Hi XXXX ! Good evening. I am Dr Shareef answering your query. I would like to know if you eat frequently from outside. If you do, you are advised to avoid this habit as infection could travel into your intestines possibly from unhygienic way of cooking food outside your home. If I were your doctor, I would go for a routine stool test to exclude any ova, and cyst. If amebiasis is diagnosed, a course of metronidazole along with a probiotc and a proton pump inhibitor would relieve you of your symptoms. If there was bacteria in the stool, a culture sensitivity of the stool would determine about the antibiotic to be started. To these medications, I would also add an anti emetic, and a bland diet with freshly made curd. If you eat from outside specially fast food, I would also exclude gluten sensitivity due to the intolerance to gluten found in such food. This has to be further investigated by a gastro enterologist, and a dietician, by a trial of excluding such diet from your routine food habits, and assessing the effect. I would diagnose you then as a case of Irritable bowel syndrome only when these aspects are excluded by clinical examination and investigations. Hi, Thank you for your response. I do not cook my food outdoors. Very rarely do I do this and its been almost a year since I did. As I mentioned before this has been an ongoing issue with me for years. I want to know if my symptoms are in fact IBS related. My dr did diagnose me with this about 5 years ago. About a year ago I was tested for bacteria/infection and tests were negative for anything like that.

Regrets for the delay in responding as it was wee hours of the night in this part of the globe when your query was mailed. Any kind of symptoms relating to the gastro intestinal tract specially the intestinal hurry, bloating, etc as in your case could ofcourse be related to the IBS as diagnosed by your doctor sometimes back. However, as I already told you, I would diagnose an IBS only after excluding other organic causes after a detailed investigations by a gastro enterologist like a upper and lower GI endoscopy specially with a history of acid reflux, and a trial exclusion of some constituents of your diet in consultation with the dietician. I would not be reluctant to go for a thyroid function test and would like to exclude any anxiety or stress factor in your case. Till then I would advise you to add fresh curd/ a probiotic to your regular diet and consult on these aspects with your treating doctor. Good luck and take care.
