Question: I have chronically high AST and ALT levels that jump between being mildly elevated to 2x normal range. Recently a GGT test was done which showed levels 8 times the normal range. TIBC was also elevated. Hep panel was negative, all autoimmune markers were negative, and all other iron tests were normal. Ultrasound and doppler ultrasound were normal. FibroScan showed mild scarring. My menses are very irregular. I have small eczema patches around my eyebrows and above my chin. I am thin but my waist is always slightly distended. What could be occurring?

Hello

Increased ALT and GGT may indicate liver injury.It may be due to many reasons like hepatitis,alcohol intake,altered lipid profile,auto immune causes,drug toxicity etc.
It is important to know about alcohol history.

AST is non specific and it increases in many conditions.

You may need few more investigations like routine hemogram,random blood sugar,lipid profile,renal function test,alkaline phosphatase,bilirubin level etc.
You may need biopsy of liver as many investigations are negative.

I suggest tablet ursodeoxycholic acid 300 mg twice daily for three months to my patients.It helps in regeneration of liver cells.

Increased TIBC may be due to iron deficiency anemia,liver injury etc.
