What Causes Burning On Penis Tip Along With Burning Urination?

Question: Hello. 7 weeks ago I had sex with sex worker. I did use a condom. However, I did perform about 1 minute of Cunnilingus on her clitorus only. 4-5 days post sex I had some pain in my testicle, some burning in tip of my penis, some burning during urination and urination frequency. I went to my urologist at 6 days post sex and had urine test completed which came back negative for and STD's. He diagnosed me with epididymitis and prescribed me cipro. The cipro did not do the job. I am still having some ,but not as much discomfort in the pelvic region, some itching/burning in tip of penis or urethra area, a little discomfort after ejaculation. But i have not had any discharge from penis at all. Here are my questions....

1. Do you think I picked up an STD?
2. Are the STD tests for Chlamydia & Ghonorrhea accurate at 6 days post exposure?
3. Could I have prostatitis instead of and STD and the Cipro not affecting it?
4. I have also been taking Bactrim and the symptoms do seem to go away/lessen as I take the meds.
4. What would you recommend I do?

I understand your concern and situation.

You are having Epididymitis which is an inflammation of the anatomical structured of males coiled tube called epididymis at the back of the testicle that stores and carries sperm. It is mostly caused by a bacterial infection, including sexually transmitted infections (STIs), such as gonorrhea or chlamydia. Your symptoms like pain in testes and urethra and frequency of urination and pelvic discomfort are suggesting epididmytis or epididymo-orchitis suggest you have picked an STD infection. Rectal examination will reveal if prostatitis. It is may be due to chlamydia, gonorrhea and rarely Tb. I suggest you to repeat urine examination at three consecutive days and send for gram staining microscopy and culture sensitivity of aerobic and anaerobic bacteria. Get leucocyte esterase test done. If scrotum tenderness increases you can get radionuclide scanning. You can take urethral swab and get tested by nucleic acid amplification technique for chlamydia and neisseria by PCR which is nearly 95% sensitive in early course of infection after 7 days of contracting infection. It is rapid and very sensitive. Don't worry sometimes it takes weeks for tenderness to go and about 3 months for complete cure. You can apply ice packs, athletic support for scrotum and analgesic. If symptoms are worsening you can again take a course of antibiotics ceftriaxone 250mg single dose and doxycycline 100mg twice daily for10 days. Abstain from sex until symptoms resolve and take your current treatment.
