What Causes Breathing Difficulty With Chest Cramps And Tightness?
Question: Hello sir,
I am 27 years and I am having breathing trouble for few weeks now also felt chest cramps and tightness. Did xray and ct scan was clear and was treated for lung infection. Still I am facing issue for shortness of breathe also had an ecg today which suggested probably having Left anterior fascicular block. What could be the issue could you please help.

Hello , I am Dr. Mody and I will be addressing your concern.
As I start I would like to reassure to that there is no problem with your heart. Left anterior hemiblock is a physiological finding. It's present in many since birth and you need not worry about the same.
It's like this everyone has a different face, similarly ecg is the face of your heart so it may be different in different ppl, but that doesn't mean it's abnormal.
So you need not evaluate further cardiac cause based on the ecg alone. Also you may upload the ecg so I can interpret it for you.
All the symptoms you describe, You may still have upper respiratory tract infection and most probably viral infection, where x ray and CT may be normal. So the treatment you are on will suffice.
Apart from deriphylline, you may take tab allegra 150mg two times, to decrease the allergic component. May add an antibiotic sos depending upon the symptoms and the place you are from.

Yes the ecg has left anterior gemiblock and sinus tachycardia (which may be due to lung infection) also many a times the changes of fasicular block may be tachycardia related and will nomalise when the heart rate comes back to normal , per say you do not require further cardiac evaluation at 27 years of age. However if convenient you may get a A 2D echo first and if normal, a treadmill stress test after reading recovery of the viral infection, because of the doubt persisting in lead 3 and avf. That will complete cardiac evaluation. It should be done if you smoke, are overweight, have sedentary lifestyle and have a strong family history of heart disease just as baseline investigation
Now for the symptoms you have at present some muscle soreness would persist for a week or two, it may be due to viral infection itself. You may take tab dolo (650) sos if pain is causing a problem in your daily activities. It's plain paracetamol and would not have any major side effect.
Let me know your follow up report s of you do visit your cardiologist

Hello Sir,
I had an appointment with my cardiologist and also done a general blood test and echo test. Reports are attached here with. Sir the condition that you have mentioned is it something serious or do i need to take precautionary measures for it. As for my habits I used to smoke for last 2 year but its been almost 2 months now that I have quit smoking.
As per my cardiologist suggestion The heart condition is normal and as for my breathing issue my pulmonogist has prescribed esiflo 250 inhaler daily twice. Telekast 1/2 tab daily for 1 month, Sompraz 20 daliy twice and Miracort 6 tablet daily twice for 4 days.
Is something else that needs to be done from my end to regain my physical health back to normal.

Now just for your reassurance, let me tell you that there is no problem in your general life because of MVP or ecg variant. Rather it's so common that it is present in one out of four population.
As I said the present symptoms were because of your lung infection and had nothing to do with your heart. As soon as it is resolved you will be back to normal.
From my side echo once in 3-5 years to keep a tab on MVP.
Lastly because of your lung, try quitting smoking once and for all. You are young and will be able to do so.
Have a healthy life and you should be doing great.
