What Causes Bleeding When On Birth Control Pills?

Question: I hemorrhaged internally when I gave birth 11 month ago and had to have a d and c. I spotted occasionally while I nursed. I quit nursing the beginning o March. My period returned just over 3 weeks ago. I started the pill and have been bleeding on and off for the last 3 weeks and it is fairly clotty when I do. I have pcos also. Not to sure if I should be concerned or not.
