Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

I passed carefully through your question and would explain that your symptoms are not related to any cardiac causes.

These symptoms could be related to your actual therapy adverse effects or an electrolyte imbalance.

Let me explain as follows:

-Carvedilol can cause a low physical performance of the muscles. This is a common adverse effect of all betablockers.

-Lipitor can cause muscular pain and an increase in liver and muscular enzymes. But these adverse effects usually last through the day.

- Losartan can lead to potassium plasma changes, triggering this symptomatology.

- Flomax is known to cause joint pain as a common adverse effect (encountered in up to 12%of the patients).

Coming to this point, I would recommend consulting with your attending physician and performing some tests:

- complete blood count, PCR, ESR for inflammation
- blood electrolytes for possible imbalance
- liver and muscular enzyme levels.

If liver and muscular enzyme levels result elevated, I would recommend reducing the dose of Lipitor or even stop it for a couple of weeks and than restart in lower doses.

Regarding the blood pressure values, which are high during the night, I would recommend making some changes in your actual antihypertensive therapy:

- Divide the dose of carvedilol morning and night half a tablet. This will also help improve your physical performance during the day and maybe improve the pain in the upper and lower arms.
- add another tablet of losartan 50mg at night (so take 50mg twice daily).

You should discuss with your doctor on the above issues!

Hope to have been helpful!

I take Losartan 50 + Coreg 6.25 in the morning. I do the same thing at night.
I have a mild pressure in my left chest throughout the day. In the morning I feel okay.
I will stop Lipitor until I see my Cardiologist next week.

Sometimes, 1 hour after medicine at night BP goes to 155/80/52. I have always low pulse rate of 59.

Any other advice?

Detailed Answer:
Hello again!

Thank you for the additional information!

Coming to this point, I would recommend adding a calcium channel blocker to your actual antihypertensive therapy (amlodipine, felodipine, etc.) to have a better control of your blood pressure during the night.

I would recommend starting with a low dose of 5mg of amlodipine and increase slowly based on your blood pressure values.

Regarding the mild pressure in the chest, I would recommend performing these tests:

- a chest X ray study
- inflammation tests (PCR, ESR, complete blood count)
- blood electrolytes
- liver function tests and muscular enzymes.

I would also recommend consider the possibility of joint pain as an adverse effect of Flomax.

Some lifestyle modifications would also help have a better control of your blood pressure:

- reduce salt and caffeine intake
- perform a lot of physical activity (Yoga, aerobics and walking are recommended)
- try to lose some pounds if you are overweight.
You should discuss with your doctor on the above issues.

Hope you will find this answer helpful!

Let me know about everything!

Wishing all the best,

Dr. Iliri
