What Causes Anxiety, Insomnia, Constant Shaking And Dizziness When Diagnosed With Thyroiditis?

Question: I was diagnosed with thyroiditis over a year ago- after 8 months of excruciating anxiety, panic, insomnia, shaking. Tsh was <.1. T3 and t4 were normal and tpo was 166. Was put on low dose thyroxine 3 weeks later when tsh came up above normal range. Tpo never rechecked, no other tests done. Have been on a roller coaster ride since with horrible hyper and hypo symptoms, endo says labs fine so can't be my thyroid. I'm at my wits end with strange neurological sensations, anxiety, insomnia, shaking, dizziness, GI upset that all comes and goes with no trigger. Please help any suggestions would be greatly appreciated. All docs I've seen say doesn't seem to be psychosomatic and that there's "something complex" going on. But 6 month wait list for neurology.

Very sorry to note your symptoms. They are most distressing. I suggest testing for an uncommon condition called Pheochromocytoma. It can result in symptoms like the ones you are experiencing. A blood test called Plasma Free Metanephrines is a good way to start screening for this. This test does not require any fasting and so it can be done at any time of the day. It is quite likely your endocrinologist has checked routine things already such as CBC, HbA1c, Glucoses, Calcium, Electrolytes, Liver and Kidney function tests. If not it would be worthwhile to do these too.

Thank you. I have had a test called catecholamine fractionation, free, urine 24 hour. Would this be indicative of a pheochromocytoma? Also, i have vision disturbances during these episodes. Ive checked my bp and while it doea rise when im having an episode, its never over say 121/85. I do have a history of severe preeclampsia but it resolved a few months after delivery. Is it possible for anxiety to cause neurologic symptoms such as foggy head woozy like sensations like im on a boat? When it gets bad i feel like i make jerky movements- though im not sure i really do. And then i go into a full blown panic attack. I know its normal to doubt its just anxiety because i do think i have a degree of hypochondria but is it possible that it could all be related to stress/anxiety?

1 Yes stress and anxiety can cause severe symptoms at times.

2 What is your current levothyroxine dose, and what are the results of your most recent TSH and Free T4 blood tests?
Taking excessive thyroid hormone at doses higher than necessary can also give rise to symptoms similar to those you are experiencing.

3 Yes the 24 hour urine test you underwent seems to be for the same condition I was referring to. I suppose it was all normal, was it not?

My last TSH was 1.0 in november and my last free T4 was 1.0 in july. Im on 0.05mg levothyroxine daily. My doctor tells me to skip it for 3 days when km feeling shaky anxious etc but it doesnt work. Last night was awful, every time i would try to sleep i would get surges. Thats the only way i know to describe it. Like surges of electricity or adrenaline shooting through my chest and head. At 3am i woke up feeling completely overwhelmed and like my head was going to explode. I took a clonazepam and was finally able to at least rest from 530-630. I feel so lost im not sure what in the world is going on. I took myself to the emergency room 5 days ago for strange feelings in my chest that were taking my breath away. Along with dizziness and diarrhea. They did bloodwork and ultrasounded my gallbladder. Nothing found. I had diarrhea with this episode last night too. I just felt dismissed by my endo and other doctors. I feel so helpless.
