I have read the present complaints, your past medical history and medication tried.
The pain you explained is commonly seen in infection of the kidney due to UTI. The pain on the right side of abdomen, around belly button- radiating to the right groin, leg and low back explains this. You also have frequency of urination, nausea indicating a need to have a rest and urgent investigations to understand the extent of the problem you have. Do no go for work.
The swollen abdomen is usually due to reflex mechanism of the body that the intestines slow down and the patient gets bloating of the abdomen.
There is a possibility of a stone on the right side.
I would suggest you the following:
First of all -send the urine for culture and sensitivity although you are already be on antibiotics.
Blood- CBC to know the WBC count for extent of infection, Sugar, blood culture and sensitivity as there is a chance of septicemia. Urea and creatinine (Kidney functions).
Urine - as said culture and sensitivity and routine and microscopy.
Ultrasonography of the abdomen- to know the extent of infection, hydronephrosis, stone etc.
Continue antibiotic, may need additional or changed ones as the previous have not helped you much.
Plenty of oral fluids if tolerated.
Admission to Hospital- intravenous fluids and antibiotics, symptomatic and supportive treatment.
Continue treatment for your Medical problems (Schizophrenia depression epilepsy and acid reflux) under Doctor's supervision.
