What Causes Abdominal Cramps And Bloating?

Question: about three or four times a year toward the end of my menstrual cycle I will have the sudden urge to urinate and can barely make it to the bathroom. After I finally make it and urinate a burning sensation will start at the top of my vagina and work it's way up to my stomach, putting me in severe almost unbearable pain. I will usually get in the shower right away and let the hot water run on my stomach for about thirty minutes which helps the pain tremendously. I will then lay down but my stomach will fill with cramps and will be so bloated I cant fit my pants. I can hardly walk or cough without pain in my lower stomach. This cramping, bloating and pain will continue for about three days. I also have chills.

2. Endometriosis of the bladder. This can cause the severe pain in the pelvis and perineum. Hormones and anti-inflammatory drugs are the mainstay of medical management.

3. Interstitial cystitis. This is a type of bladder inflammation not caused by virus or bacteria but by a leaky lining membrane of the bladder. Thus, urine leaks into the inner layers of the bladder wall causing inflammatory reaction.

4. Dysmenorrhea. This is less common at your age.

A proper clinical evaluation, a sonogram of the abdomen and pelvis, preferably in the trans-vaginal route, a renal scan to look for any anatomical causes or pathology, urine culture and general evaluation are needed. I would ask you to see a urogynecologist for proper appraisal of your situation.

As investigations are reported, the situation is clarified and further management can be planned.

Management of weight, exercise, strengthening of the pelvic floor through Kegel's exercises, bladder training etc., are some helpful measures. Use of antispasmodics can help get relief from the spasm.
