What Casues Light-headedness In Certain Sitting Positions?

Question: Hi Doctor,

I get lightheaded/dizzy in certain sitting positions (in chair) as well as immediately after lying down. These spells do not last for a long time, but go off soon.

The ENT doctor did all the tests and did not find any problems. I visited an Ortho as per my family physician advice. The Ortho did a neck X-ray and mentioned that I have spondolysis. He has given me Vertin for 2 weeks and asked me to start on Physio Therapy.

My question is that can spondolysis cause light headedness even in certain sitting positions. Also, can spondolysis be cured by exercise (I exercise regularly) and physio therapy?

Since when are you on Nebicard ? How much does your Blood pressure range within ?

You seem to be having postural dizziness especially during sitting and on lying down. This can occur in a variety of conditions like blood pressure fluctuations, low blood pressure, increased stress, dehydration, middle and Inner ear disorders and only advanced cases of Cervical spondylosis.

In response to your question - Yes, Cervical spondylosis can cause dizziness BUT only in advanced cases where lot of bone spurs around the vertebral bones compress and put pressure on the vital arteries that passes nearby. This can be aggravated on turning and rotational neck movements.

In your case, it is only mild to moderate Cervical spondylosis, Therefore I believe cervical spondylosis should not be the cause for your dizziness. I have reviewed your Neck X-ray and noticed only few spurs and not much reduction in the disc space, placing you at mild to moderate stage of Cervical spondylosis.

Accordingly after ruling against cervical spondylosis, I would like to concentrate on other possibilities:
1. I would like to mention that the medicine Nebicard or Nebivolol is known to cause dizziness as a side effect and can also cause postural dizziness.
2. There is also a condition called Orthostatic hypotension, wherin the person experiences fall in blood pressure by atleast 20/10mmHg (Systolic B.P./Diastolic B.P.) after he gets up from the lying down position within 3 minutes of doing so.

I advice you to get your blood pressure recorded during lying down and sitting positions separately to see how much the blood pressure fluctuates. This is an office tests done at your physician's clinic. Also, discuss about whether or not Nebicard is affecting your blood pressure leading to dizziness.

As far as Cervical spondylosis, I can understand and empathise with the high stress lifestyle you may be living. Avoid working long hours in front of the computer. Try to find time for yourself to relax. Do deep breathing exercise, regular brisk walking for 45 minutes a day and Yoga for effective stress relief and maintain fitness. Avoid smoking and drinking alcohol as these can also contribute significantly to blood pressure and dizziness. Eat healthy. Keep well hydrated. Be on a low salt diet due to your high blood pressure. Avoid junk food, fatty, oily and fried food.

Avoid bending forwards and rotating head to extremes as these can put strain and cause discomfort. Avoid carrying heavy weight on the back of the shoulders. You can apply hot water bottle cautiously to your regions of pain at the back of the neck and other points for pain relief. If you have neck or shoulder pain, you can take pain killers and muscle relaxant combination after discussing with your treating doctor.

Neck stretching exercises and neck physiotherapy are very helpful in reducing the symptoms of Cervical spondylosis. This combined with proper ergonomics at work will definitely help to retard the progression of Cervical spondylosis.

Thanks for your response.

My BP reading was 129/84 and I checked it last time with the physician 1 month ago. I have been able to maintain the BP around 130/85 with this medication. Regarding Nebicard side effects I will discuss with my doctor next time.

Also currently for spondolysis, the doctor has asked me to go through PT sessions for the next 10 days and then do strengthening exercises. Overall I am pretty active and I walk for 4 km everyday in the morning daily as a ritual and do not miss this.

Regarding the dizziness, I understood your point about orthostatic hypertension. But I get the dizzy feeling when I sit on a plastic chair. It sounds weird but at some position I tend to get it and I need to shift positions constantly to deal with it. It does away or reduce when I stand up and walk; but it would be impolite of me if I do this in an office meeting :). So I'm puzzled why this is happening and was curious if the spondolysis diagnosis might cause this.

I can empathise with you about this rather unusual situation.

In some persons, blood tends to pool into the legs and not travel up to the brain quick enough via the heart if you are sitting for long hours. This can cause dizziness. But once you start walking, the strong leg muscles act as a pump and push the blood right up to the head and
everything feels fine again.

I can offer you some more advice regarding this issue.

(1) When you sit on the chair, you could flex your ankles up and down for about 10-20 times slowly. This will activate the leg pump mechanism to send the blood to the head. You must do this while you are sitting on the chair, every 10-15 minutes or whenever you begin to feel dizzy.

(2) Drink 2 glasses of water before you set out to work, as this will help in the fluidity and flow of the blood and prevent it's thickening.

(3) Avoid excess caffeine, colas or smoking, as these can reduce the blood pressure or permeability of the blood to the brain.

(4) Deep breathing exercises whenever you feel dizzy will also help pull up blood to your brain via the heart pump.
