The picture isn't clear enough to tell. If it is blistering and excruciatingly painful, like electric shocks when the area is touched, then it could be shingles.
I strongly advise that you go to the urgent care clinic or ER NOW, because there is definitely an infection happening. If it looks like shingles in person, then you should start an antiviral immediately - acyclovir or Valcyclovir.
But I cannot tell from the picture - it could be a bacterial infection instead. If that is the case, hot baths, hot wet compresses to the area, and likely an antibiotic are needed.
But again, I'm sorry but I can't tell from the picture (and I do know what shingles looks like at various stages very well having had them myself!). But whatever this is needs to be seen in person and treated now.
