Question: Am 29 yrs old male from South XXXXXXX I was diagnosed with hypertrophic cardiomyopathy,my heart rate is 47.I always have low energy .i travelled to Nigeria for 10 days where I was exposed to mosquitos bites.now I have been sick for a week since I came back. I did malaria screening which is negative and the doctor refused to give me medication .
A friend of mine from Nigeria sent me coartem 80/480 which I took after few Hours I started feeling feverish dizzy fatigue .i read the leaflet then found out that it says do not take coartem if you have a heart condition ,such as changes in the rhythm or rate of the heart beat , slow heart rate or severe cardiac disease which applies to my case.
I tried to vomit it out immediately which I couldn't then I eat lots of coconut
and palm oil .
This morning I wake up and Iam feeling worse.feverish,no energy ,just total unwell.
i still have artesunate 50mg. Should I take it or will my condition get worse.
Most doctors in South XXXXXXX don't know anything, they only rely on the lab test. They run multiple tests just to waste patients money. I have been to three different doctors in a week and none could give me any thing that could relieve my symptoms

I understand your concern and would explain that your symptoms seem to be related to coartem adverse effects.

I don't recommend taking this drug any more, as your tests have resulted negative for malaria. In such case you should know that the possible adverse effects of this drug don't justify the possible benefits, especially when all the tests result normal.


Nevertheless you should not worry any more about this drug intake, as its effects will resolve spontaneously within some hours ( based on its farmacological properties).
Your symptoms may also be related to another infection, transmitted from mosquito.

From the other hand, it is necessary to evaluate once again your cardiac function.

When did you perform your last cardiac echo?

Can you please upload the reports for me to review?

I recommend consulting with the GP for a careful physical examination, a resting ECG and cardiac ultrasound, a chest X Ray study and some blood lab tests ( complete blood count, PCR, sedimentation rate, blood electrolytes, kidney and liver function tests, fasting glucose, etc.).

You should also consult with an infection disease specialist and perform further lab tests for possible infectious diseases that can be transmitted from mosquitoes.

I did blood culture and there is no growth.
My leucocyte count is 3.51 (4.0-12.0)
Neurophils 1.27 (2.0-7.5) and the rest is normal
My major problem is my very low energy level
I sleep at 10pm and wKe up at 7am everyday feeling sick with fever,no energy,total body weakness ;around 9am i will start feeling a liltle better and start my day then around 4pm the symptoms will come back like in the morning making me to feel miserable untill 8 pm .
I get sick after ejaculating both during sex and masturbating,so am avoiding sex and masturbation recently.i used to masturbate 3 to 6 times a day for 11 years but i have stopped for about 6months .
I went to the urologist last 2 months and all the test is normal and he referred me to a cardiologist who diagnosed me with hypertrophic cardiomyopathy.
I know my case is complicated but iam just being honest for the first time.My health and energy is very poor now i cant even go through my day
I have been to lots of doctor but none of them could help relieve my symptoms
Plz can you advise me on what to do from now going forward.which type of doctor can i visit that can help me

The total number of white blood cells is lower and there is an exceed in lymphocytes, which may be related to a viral infection.

I recommend consulting with an infectious disease specialist to perform some tests for possible viral infections, associated or not to mosquito transmission.

Besides, what about a tick bite?

Have you noticed any possible sign of any tick bite on your body?

Lyme disease could also lead to these chronic symptoms.

Regarding your cardiac disorder, I recommend consulting with the cardiologist and perform another ultrasound to examine your cardiac function.

Sometimes a low cardiac function could also be related to chronic fatigue.

I reviewed your uploaded photo and my opinion is that it is hard to conclude about an etiology of your complains based on that.

Facing your clinical symptomatology, your performed lab tests and your epidemiological history I strongly recommend you to consult an infectious disease specialist as a comprehensive differential diagnosis should be done.

Regarding hypertrophic cardiomyopathy and its adverse implication on sexual activity, I could explain that in patients with HCM severely symptomatic sexual activity should be deferred until their condition is stabilize.

As about 70% of hypertrophic cardiomyopathy patients may present with left ventricular outflow obstruction and also may offer an arrhythmogenic substrate, a vigorous sexual activity may increase the risk for potential complications.

That's why I recommend performing a careful cardiology checkup including a cardiac ultrasound to exclude potential cardiac dysfunction contribution in the actual clinical scenario and define the allowed level of physical activity and sex.
