What Are The Risks Of Developing HIV Post Non-penetrative Sexual Activity?

Posted on Fri, 4 Mar 2016
Question: Hi Doctor,

I went through the Health care magic website and one of the doctor in the website has mentioned that, penile rubbing on the vagina, is risk for hiv and not negligible. Please see the below link.
http://www.healthcaremagic.com/premiumquestions/Can-vaginal-fluid-enter-through-urethra-after-rubbing-penis-on-vagina-and-cause-HIV/46539#

The women just rubbed my penis against the vagina(probably minora labia) for just 4 seconds,I remember no wetness on vagina.

However, hypothetically speaking, if there was wetness on the vagina, what would be the risk of hiv from rubbing of penis on the vagina for 4 seconds. In thebody.com, the Doctor states that it is no risk activity. http://www.YYYY.com/YYYY/YYYY/YYYY/0000.html.

Kindly let me know if this is a negligible risk.

I will definitely go for HIV testing in the next 4 days.

Brief Answer:
As per CDC, XXXXXXX vaginal rubbing came under negligible risk category.

Detailed Answer:
Hi XXXXXXX
Welcome once again to HCM.
As far as any infectious diseases management, the medical profession of the world follow the guidelines framed by Center for Disease control, US public health department at XXXXXXX of united states of XXXXXXX As per this site, HIV transmission risk through anything other than blood transfusion, needle prick, anal vagnal ,oral intercourse, all other activities included as 'others' and the risk is declared as 'negligible' only. You can see in this web site. http://www.cdc.gov/hiv/policies/law/risk.html
We are following the same and our experince in this field for more than 30 years in my practice and also throgh various source like journals and confecrences confirm the same.

Hi Doctor,

I went for the Chemiluminescent Microparticle Immunoassay(CMIA) test after 28 days(4 weeks) post exposure. The report has come as non-reactive.

The report states that, any reactive or non-reactive report is recommended to be confirmed with additional test like HIV Western blot, HIV RNA detection by PCR method or by proving rising titre in repeat test after 1 month.

Kindly let me know what a non-reactive test refer to? Also please let me know if I have to go for repeat test after some days.

Brief Answer:
You are free from HIV 1 infection. Better repeat the test after 12 weeks.

Detailed Answer:
Hi XXXXXXX
Welcome back.

Almost all the tests available are highly sensitive and specific to the extent of more than 97%. Especially Chemiluminescent test is reliable to the extent of more 98%. No test is 100% reliable. So all the laboratories used mention like this (verify once again with WB, PCR) to safeguard themselves from legal litigation problems. Not to be taken as very serious. But two different types of HIV tests either negative or positive make its reliability to the extent up to 100%.
Regarding your HIV negative test with chemiluminescent by 28 days--- It used to have two parts: Antigen part of this test only becomes reliable and conclusive by 28 days and the antibody part can become reactive and relaible only after the window period (6-8 weeks, may be delayed upto 12 weeks).
At the same time Antigen part can detect only HIV 1 infection and not HIV 2 infection, unless it is specified, whereas the antibody part of the test can derect antibodies against both HIV 1 and 2 infection, if present or rule out in the absence of infection, but that too only becomes reactive only after window period.
So Your negative HIV test by 28 days ruled out HIV 1 infection. HIV 1 infection is almost common more than 99%.
HIV 2 infection prevalency is less than (0.5%) throughout world except in certain parts of XXXXXXX
So you are free from HIV 1 infection, better repeat the test after 12 weeks once again if you want to rule out both HIV 1 and 2.

Hi Doctor,

In disclaimer section,it is mentioned that the test is limited to qualitative detection of antibodies to HIV in Human sera. So I don't know for sure that, HIV antigen testing was done.

Is there a CMIA test without HIV antigen test? If yes, can I again undergo the test tomorrow with Antigen and Antibody combo.

Regards, XXXXXXX

Also Doctor, the following link states that the virus is detectable between 20 to 23 days of infection.

http://www.YYYY.com/YYYY/YYYY/2014/08/YYYY.png .

I will definitely do the repeat testing after 3 months. But please let me know, if I can take a repeat testing tomorrow which has both antigen and antibody detectable in the test.

Brief Answer:
COMBO test will reveal the HIV 1 status by 28 days.

Detailed Answer:
Hi,
Welcome back.
You can upload the copy of the test report, so that we can discuss about the same.
4th geneartion COMBO/DUO test can detect the HIV 1 virus antigen by one week onwards after the infection and becomes conclusive by 3-4 weeks. Antibody part of the COMBO test will be able to detect antibodies only after the window period.(after the appearnce of antibodies inside the body). So if you have the test 4th generation combo test by 4 weeks, it will reveal HIV 1 status clearly beyond any doubt. After 12 weeks it will conclusively reveal the HIV status (HIV 1 and 2) beyond any doubt because it decides by two ways (antigen and antibody detection)

Hi Doctor,

Today again I went to the Ranbaxy SRL laboratories and had a word with the MD doctor from research and diagonisis.

I have given for 3 tests, HIV early detection test, PCR test for HIV1, XXXXXXX test.

I am anticipating the result by Wednesday night. Also the microbiologist who is into this testing confirmed that, the assays reacting to HIV1 P24 Antigen has no specific component to detect HIV 2 antigen. However, since p24 antigen assay has conserved regions in gag capsid protein, p24 sensitive antigen assay can also detect the P26 HIV 2 antigen.

Please note that, I have already once done my CMIA test with KG hospitals and the results came negative.

I will definitely do the testing at 12 weeks mark. Kindly let me know the probability of getting HIV 2 virus at that time. Also please let me know what does 0.5 HIV 2 prevalance rate mean.

Brief Answer:
HIV 2 infected constitute less than 0.5% among the total HIV positives.

Detailed Answer:
Hi,
Welcome back.

I am a clinician. Microbiologist may be more expert to say more about this validity of test. If you are convinced by their statement, relax.
Among the total people who are HIV Positive, less than 0.5% are infected with HIV 2 infection in XXXXXXX In other parts of world except in certain parts of XXXXXXX the prevalence of HIV 2 infection is less than 1% only.

Brief Answer:
Upload the previous CMIA report along with newer one.

Detailed Answer:
Hi,
Welcome back.
You upload the previous CMIA result also, so that we can verify whether you had antigen test or antibody test.
It is good that you are going to have HIV 2 PCR results also. It is highly reliable test. If that test comes nonreactive it is 100% sure that you are not having HIV infection (both HIV 1 and 2).
