Question: I have a problem of piles and there is a risk of fastula developing as well. What steps should I take to affect a cure. Kindly advise medicines, ointments and other actions to be taken. Thanks

There is no risk of development of fistula in those who have piles. So need not worry on that part.
Piles is a fairly common problem and most people may my have piles even though they may not develop any symptoms through out the life time.
If you have bleeding in stools or severe perianal pain then only it needs treatment. Treatment is usually avoiding constipation, avoiding straining for stools.
Can continue applying that cream before passing stools for few days.
Seitz bath is an good treatment too so can continue on that.
Stool softners like isapgula husk can help soften stools.
