What Are The Chances Of HIV After An Unprotected Sex?

Question: I had a sexual unprotected encounter with a friend of mine and my partner. It involved oral sex and anal penetration. My partner was the bottom. My partner and I have tested negative continuously the past four years, and I took my friend to get tested after the encounter and he tested negative. He said his last sexual encounter was with a female 3 weeks ago and it was protected. Although, a male performed fellatio and analingus on him unprotected. I and my partner started PeP (Truvada and Isentress) within 24 hours of the encounter. I am now on day four of PeP, and a lymph node on the upper left side of my groin is slightly swollen and painful to the touch. Should I be concerned that this is a sign of acute infection?

You and your partner are HIV negative (Is it 3 months after your last contact for either of you with someone else?). Your friend is also nonreactive for HIV (once again has he any other unprotected contact within 3 months?). If not, it seems that your HIV risk is negligible, Even though your friend had unprotected oral sex and anilingus, he is at low risk.
Anyhow you had started PEP started within 24 hours. That is well and good and you need not worry about HIV. PEP offers protection only against HIV and not against ordinary bacterial infections and other STDs. As you had unprotected oral and anal intercourse, you are at risk of other STD. So consult you physician and have a thorough examination and investigations to rule out STDs. He/she will guide you for the treatment of lymph node swelling after the examination and investigation.

No, my partner and I were strictly monogamous, prior to this incident. My friend has stated that this was his only unprotected incident in the last 3 months.

If your partner is strictly monogamous and your friend didn't have any unprotected sex during last 3 months then the risk for HIV is negligible. Your friend's experience of felatio also was 3 months back or recently? Moreover you and your partner are on PEP. So your chance for HIV infection is almost nil after completion of PEP.
