What Are Side Effects Of Dianorm And Istamet?

Question: I am 32 years old and I am a diabetic, about two months back when I checked my blood glucose level it was 171 Fasting and 351 PP. I have been advised dianorm 80 (2 tabs ,30 mins before food) and ISTAMET 50/1000, 30 mins after food. I have been taking this regularly for the last 1.5 months and now my fasting sugar has come to 120. Also I am having vingose 0.3 with 1st morsel of lunch, However 2 hours post lunch my sugar level are close to 220, (although I did not check after breakfast, i.e when I have istamet and dianorm), my concern is how long should I continue these medicines (Life long ?) , What are the side effects this has on my body, any care I should be taking apart from regular blood tests ? , Ever since I have been having these medicines, I have very high acidity, should I also be taking acidity medicines also, if yes, what would it be ? , Is it recommended to have some ayurvedic supplements also along with allopathic medicines which might help reverse my present condition ?

Hi, I had gone through your query and understood your concerns.

Sir, First let me appreciate you for your positive approach.
Diabetes is a condition where blood sugar levels are high and the medication to control them should be continued as long as they under control. After some time if blood sugar levels are persistently high even with that medication, then we need to increase the dose under guidance of treating doctor.

The sugar levels you have mentioned are slightly higher to good control, but not to worry much. The anti diabetic drugs you are taking are very effective. You can continue same medication for some more time and see. Once the doses are set to control sugar levels, then you can discuss with treating doctor and take combination of drugs, so that number gets reduced. You need to take medication life long. The are no considerable side effects for these drugs.

Acidity may be a coincidence. Any how, you can try stopping them and see a while, with no spicy food, no empty stomach, taking milk products etc.,

There are no medicines which could reverse the condition. Any medicine could only help us to control the sugar levels. With sugar levels under control, you can lead very much normal life as a non diabetic individual.

The other care you have to take in diabetes is regarding diet and exercise.

Diet:

Avoid single high concentrated meals.
Small frequent meals more times in a day.
Food high in fibre content like vegetables, whole pulses, fruits, etc.
Ideal combination of diabetic diet should be:--
Carbohydrates: 55-60%.
Fat: 30%.
Proteins: 15-20% (0.8g /kg /body wt).
Fibres 40 g/day.
Take unrestricted:--
Salads (cucumber, tomato, onion, raddish, salad leaves).
Vegetables (cabbage, baigan, lady fingers, french beans, methi, palak).
Fruits (except banana, chockoo or mangoes).
Sprouted beans and pulses. XXXXXXX biscuit (with less sugar).
Clear soups, Lemon juice, Buttermilk

A 40 minute physical activity every day like morning walk, evening walk or a visit to the gym or swimming pool.

Thank You doctor, I am a little curious now to understand this better, How will the medicines act on my body, for eg. I am taking Dianorm and Istamet at night, for how long does it's effect last, I have seen that in the afternoon after I have vingose and check my sugarlevels after 2 hrs it is normally high, can I take a risk of taking 2tabs Dianorm and 1 tab ISTAMET in the night and then one tab of vingose in the morning and 2tabs Dianorm and 1 tab ISTAMET in the afternoon. I will keep monitoring my sugar levels during this activity.

Hi, I had gone through your query and understood your concerns.

Sir, Different classes of drugs act in different ways, For eg. Dianorm acts on pancreas and helps in increase in insulin production. Istamet is a combination of sitagliptin and metformin of which metformin acts by reducing release of glucose from liver and sitagliptin is dpp IV inhibitor. Voglibose acts on post prandial sugar. What ever the route they take, all together act in reducing the blood sugar levels.

They are prescribed depending on time of action and dose required so that they reduce blood sugar levels and at the same time does not cause hypoglycemia which very low blood sugars. Most of them have an average action upto 12 hrs. Some slightly lesser and some upto 24hrs.

I forbid you against doing any changes / trials with your antidiabetic medications. That should be absolutely done under clear instruction of treating doctor after checking sugar levels in order to prevent complications.
As such I am ok with your current regimen. Instead of you doing drug changes, I suggest you continue the same for couple of month. Blood sugar can be assess 2 months post treatment. If the sugars are not in control, your doctors will modify the regimen.
