Question: Today woke with abdominal pain, cramping, significant burping, some flatulence but more than normal. Attempt at vomiting once but not successful. I am Diabetic, hypertensive, hypothyroid. I am on Zantic b.i.d for severe stomach that follows taking my generic Prozac. Did have Gastric Bypass in 1987 and can easily compensate if there is a problem. It believe there must be significant flatulence (not yet expressed) as both breasts are painful, I have had diet 7 up mostly today. Eating some Rice Chex but no milk or sugar, I did have some Progresso Chicken Noodle but but could only drink that slowly. I am trying to make sure I stay hydrated.

Noted the history, medications and understood the concerns.
Noted all the medications you are taking.
The causes for waking up with pain in abdomen, cramping, significant burping and some flatulence can be:
Gastritis.
Small intestinal problems.
Side effects of any medications.
You had Gastric bypass way back in 1987, hence it is better to get the present status of surgery.
I would advise you the following in such a situation:
X-ray of abdomen in standing position.
Symptomatic treatment with Domperidone, you may need a prescription. Domperidone will help with burping.
Take an antacid.
You are already taking Zantac BID, please continue. Additional PPI like Omeprazole can be added if needed after symptomatic treatment.

Further investigations like Ultrasonography of abdomen, upper GI Endoscopy as needed after consulting your Doctor after clinical evaluation and examination.
All these will help to get an exact cause and thus help in proper management.
Antispasmodic as you were woken up with pain (means the pain may be severe).
Please avoid induced vomiting as this can add to esophagitis and GERD like symptoms.

Revision of the medications, if any of these is suspected to cause the symptoms. Lesser the medications better are the results. For example you are taking Prozac, Wellbutrin ER, Ambien 5 mg every 3rd night, Melatonin : your Doctor can surely help to reduce the medications or dosages as many of these have similar actions. You can not reduce or alter the dosages on your own, needs to be done by your Doctor.

Antacids can be taken on empty stomach when you have not taken any other medications. And only a few times.
