Question: I am 45, and have gained about 20 pounds in 6 months. I took phentermine 37.5 a few years back and had great success. Was looking for a weigh loss plan like that to start. No allergies or health issues other than the start of menopause.

First I would like to know your height and current weight?

Have you ever checked your thyroid hormonal levels? If not, I suggest you get thyroid profile done. If yes, then please upload the reports done till now.

Yes, perimenopausal can cause hormonal changes in body and can cause weight gain. You have to maintain your diet and should do physical exercises ( I appreciate that you are already doing same).
I suggest you brisk walk for 30 minutes. Instead of walking you can do aerobics.
Eat small meals 4-5 times a day.
Maintain an interval of 2 hrs inbetween meals and do not have anything. Do not sit or lie down immediately after food, walk for 10 minutes.

Regarding phentermine: Here I suggest you first you try with diet and exercises for few months. If you will not see results then you can start with low dosage or previous dosage of phentermine (Since you had success with it and you do not have any health issues) after consulting a doctor. Dosage will be decided by your doctor according to your weight.
