Question: Hi there, so today I had a zit on the bottom of my acorn it looked like a fordyce spot encircled by red. It didnt hurt or anything but I touched it and it popped out alone. So my question is what is this? I send pictures of the after results. It doesnt hurt burn itch or anything. Is this a kind of enflammed fordyce spot or something else? On the pictures you can only see the after results, after it popped out. Please consider every possibility, thank you

I have gone through your query and I have also reviewed the Image.

In the Image I can see a single, small erosion/ break on genital skin.

I would like to rule out possibility of genital herpes. Genital herpes is an STD and usually presents with grouped, discrete, erosions on genital skin.
Herpes infection is usually acquired by direct/ intimate contact with an infected sexual partner i.e either vaginal, anal Or oral sex.

Are you sexually active?If yes, with single or multiple sexual partners? When was your last sexual contact?

I suggest a blood test for herpes i.e ELISA for type 1 & type 2 Hsv, IgG and IgM.

I have had the same sexual partner for 3 and a half years. And as I said the zit didnt hurt or burn or itch. Witch herpes does. I never had herpes before the first outbreak should be painfull with more blisters so couldn t it be something else ?

I am not yet confirming it as herpes. It is just a possibility and a blood test would be either confirmatory Or rule it out altogether.
Genital herpes, though always characterized by genital sores, is varied in its symptoms/ clinical presentation. It may be very symptomatic, specially if it is a new infection Or it may go unnoticed, specially if it is an old infection/ recurrence.
Therefore, presence or absence of symptoms in not always a reliable guide.
Moreover genital Hsv type 1 infections are usually mild as compared to Hsv type 2 infections.

First clincal episode is usually symptomatic, specially if it happens soon after the infection is first acquired.
However, new infection may go silent and when the infection manifests months or years later, it may not be very symptomatic because of the presence of already circulating antibodies.
Genital herpes may remain clinically silent for months and patients may be unaware of the infection and may not know who did they get it from, if they have had different sexual partners in the past.

Besides genital herpes, the only other possible cause could be that it is probably an inflammed fordyce spot which does not require any active intervention.

Hello doctor thank you for all that info. I have more information for you, my penis skin is a bit peeling and my acorn feels dry. Sometimes I feel like me penis skin is stretching, is this a mycose? Would the zit be connected to the mycose? The top of my acorn looks a bit pale. If the zit is not connected to the mycose then I think it is fordyce spot but I will do the test for hsv 1 and 2 when I am back in my country because now I am on holiday. Ho and by the way this couldn't be syphilis right? ( I am scared very fast :p)

No, it does not look like a syphilitic sore.
Peeling skin and dryness could be due to a yeast/ candidal infection.
However, on reviewing Image this erosion does not look like it has anything to do with a fungal infection.
If not herpes this is most likely an inflammed fordyce spot. You may use an OTC topical antibacterial e.g 2% muprocin ointment, twice daily.

So yesterday after having sexy time with my girlfriend I had this on my penis (see 3 new pictures). We went kind of hard during the sex. Is this wound due to hardness or do you recognize any std ? ( the wound doesnt hurt at all)

I don't think that it is due to trauma. This looks very much like a herpetic sore, even more closely than the previous one.
All the more reason that you should get tested for genital herpes i.e ELISA for Hsv type 1 & type 2, IgG and IgM.
