Suggest Treatment For Wheezing And Breathlessness In Child

Question: Hi,

My daughter is 1 year 6 month had 4 time. She had wheezing issues 4 times since she was 8 months old. Each time she was asked to take Duolin and Buedocort nebulization. Now doctor has asked to take Buedocort 100 inhaler 1 puff twice a day for 2 week. Tonight again my daughter is breathing fast inspite of gv



wheezing issue since her birth. Last wheezing she has shortness of breath. Doctor immediately gave her Duolin and buedocort nebulization with oxygen and measured her oxygen level. Before the medicine it was 85% saturation level and after medicine it was 96% saturation level. Doctor asked her to take Duolin and Buedocort respule nebulization for 5 days and meet her. After 5 days doctor said that she is putting her on buedocort inhaler 100 for 2 weeks as preventive measure.
Is is ok to give inhaler for 1 year 6 months old kid.
She also asked to consult peadiatic pulmonologist.

Hi...I understand your concern. By what you have quoted I think that your kid is having a multi triggered wheeze. At this age we should not put the label as Asthma as kids can grow out of this wheezing tendency by 5-6 years of age.

With 4 episodes occurring as since 8 months of age and the recent episode causing low oxygen saturation levels usage of Budecort is 100% justified. Your pediatrician is right in doing so. The medications suggested by your pediatrician are correct.

Suggestions:
1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot wheezing is 100% controllable.

2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your paediatrician for this.

3. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.

Hi Doctor,

Thanks for your reply. It gave me relief that my daughter is on correct medication.


Today when we went to doctor, for fast breathing they gave her nebulization beudocort and duolin. After that my daughter was ok. Doctor asked to continue nuebulization for 3 days (Beudocort 3 times and Duolin 2 times) and gave omnacortil syrup (3 days 2 timees 4ml). After that asked to visit them.


They also said, we may need to continue beudocort for longer duration since every time my daughter get cold/cough it is accompained by wheezing.

Beudocort be steroid I am worried is it safe for longer use...

Hi....even though it's a steroid it's in microgram dose that is delivered... So the side effects are kept to the very most minimum.

In medicine any drug will have its own side effects.... Either minor or major. But as doctors we are trained to look for risk benefit ratio keenly and decide whether to start a medicine or not. When the benefits out weigh the risks we definitely start the medicine.
