Suggest Treatment For Viral Illness With Chest Congestion And Blocked Nose

Question: My 8 year old daughter came back from school on Friday with dry cough and developed a fever of grade 101 F soon after. The entire Saturday she had a very heavily congested chest( heavy wheezing) , blocked nose( terrible blockage at bridge). Her fever has been going upto 102+.
She has had a similar case of Rhinitis with wheezing before.
I have started her on the last prescription for similar symptoms. I would like to talk to a specialist doctor to have his opinion on the medication that I have begun.

Hi....By what you quote this seems to be like a viral illness with chest congestion and blocked nose or it could be an allergic bronchitis or mild intermittent asthma. I have a few questions for you -

Questions: Answer these questions keeping in mind her health status for the past 6 months.

1. How many days per month does she cough or feel breathless?
2. How many nights per month does her sleep get disturbed due to above symptoms?
3. Does she feel breathless when she runs around or plays with other kids?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?
7. Has she been stared on inhaler therapy anytime before?
8. What was her age when she 1st started to wheeze?

Kindly get back to me with answers to above questions and I will be glad to help you.

1. How many days per month does she cough or feel breathless?
Not every month, but in XXXXXXX she has wheezing symptoms every 2-3 months. Please note that it is wheezing and not breathlessness.

2. How many nights per month does her sleep get disturbed due to above symptoms?
She does not have wheezing that frequently as I mentioned above, but she has very regular symptoms of blocked nose each night that disturbs her sleep each night.

3. Does she feel breathless when she runs around or plays with other kids?
No, never. Even in episodes of high wheezing she has the energy and willingness to play and does not feel breathless.

4. Are the symptoms when there are seasonal changes?
Yes.

5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
Yes, her mother has mild asthma. May be once in a year episode.

6. Is the cough always associated with fever?
No.

7. Has she been stared on inhaler therapy anytime before?
Yes she was started. But a change in place (to Singapore) helped her and there was not even a single episode in 2 years. So in the last 4 years she has not been taking anything regularly only symptomatically.

8. What was her age when she 1st started to wheeze?
Under 1 year.

It requires inhaled steroids

Hi....this looks like moderate persistent asthma by history. I suggest you see your nearest pediatric pulmonologist.
If I were treating her, I would prefer Foracort200, levolin, Monteleukast.
I would stop oral medicines and shift her to metered dose inhaler.
