Suggest Treatment For Viral Fever In A 5 Year Old Child

Question: My son is 5 years.
Weighs 21kg.
On Sunday morning he had a temperature of 100.
We gave him meftal every six hours.
On Monday, we took him to his pediatrician.
He said that it is a viral fever and given the below:
Meftal - P 7.5ml every 6 hours
Sinarest - 5ml 3 times a day
LCF Kid - 5ml 3 times a day
Augpen - 4ml 2.5ml 4ml
Tueday the temperature went upto 102.7. Even after giving the medicines, the temperature would come down to 99.5 and then start spiking up to 103.
The doctor then stopped meftal and started ibugesic plus. Also he said that if the temperature persists we should do a blood test for Dengue.
Now, after ibugesic, he sweats and the temperature comes down.
The problem is that every four hours his temperature goes upto 102.
Physically he is active and his appetite is normal.
Question how long should we wait? If the fever persists, when should we get a blood test done

I completely understand your concerns but don't worry. Many a times viral fever can have this type of presentation. Since there is no other symptom and the child looks fine otherwise, you may wait for 2 more days.
If fever spikes worsen, other symptoms appear or even if spikes persists, then you may go for the following laboratory work up:

- complete blood count (CBC) and peripheral smear examination
- C reactive proteins( CRP)
- TYPHIDOT M test to rule out typhoid
- rapid malaria test.
- Chest X Ray
- Urine routine

Dengue test required only if CBC show low platelet counts.
For the time being just continue the present treatment.
