Suggest Treatment For Tumor In The Brain

Question: making sure that i am covered until february 12

rom this health problem described I have been diagnosed with a tumor on the brain. just wondering what standard procedures follows this diagnosis

I am getting a strange feeling in my body. It starts in the mouth. Randomly I get a feeling maybe like I have just swallowed pepto bismol. This could be coming from my sinuses , can't really determine. But it is in my mouth , immediately fades away. As it fades I get a cold chilly feeling on the left side of my body from shoulder to the feet. As this chill fades away I get a hot feeling on the left of side of face especially surrounding the ear which immediately fades away. These strange feeling occurs about every 45 to 60mins. I am very concerned about what's going on. I first got this taste upon swallowing a protein shake that had been in refrig for at least 24 hrs. When I took one swallow I notice that it had not gotten cold at all. As a matter of fact it was hot. My mouth filled with thisPepto bismol taste or feeling I threw the remaining in the trash. About three days later I started getting these feelings that I described above continuously. This has been going on for a month now. Saw doctor and she treated me for sinus infection. This has not helped

2.Is there any Urinary discomfort?

There is no abdomen pain. The only urinary discomfort is frequently which was present before the strange symptoms. I would say ño to nausea and definitely no vomiting.

Considering your latest responses , there seem to be no immediate threat or dysfunction to your body systems.

All you require is a thorough screening if your renal system for symptoms of urinary discomfort.

For this , I suggest an ultrasound abdomen and pelvis with Complete Urine Examination.

Further management if any is largely based on test results.

I do not understand this statement "All you require is a thorough screening if your renal system for symptoms of urinary discomfort." Could you explain more closely.

Just wondering if Icould have some kind of bacteria infection. Sepsis?

I must say that reading the question was a little confusing as you have inserted all your past conversations on this site. So if I have understood things correctly the abnormal sensations have been interpreted by your doctors as possible seizures, or they found some abnormal neurological signs which prompted them to require an MRI.

The MRI has put as the most likely hypothesis a tumor, followed by stroke or multiple sclerosis (MS).

MS is very unlikely at your age so I would discard it right away.
A stroke is a commonly occurring possibility, however judging from your symptoms and the MRI report I would agree that a tumor is more likely.

As to the next step I believe it should be a neurosurgical consult as most probably (unless the neurosurgeon disagrees with the MRI report) surgery will be next. Since the primary hypothesis is a metastasis, a tumor coming from somewhere else, then some blood tests for tumor markers as well as a thorax and abdomen CT/MRI scan looking for hidden tumors will be done as well. Surgery will consist in a biopsy of the affected tissue to confirm it's a tumor and if that is confirmed to be the case to determine also the type of tumor (MRI can give indications but is not enough to determine the exact type of tumor).

Further management, whether you will need chemotherapy or radiotherapy will depend on the results of these tests, strategy varies according to the type of tumor.

Thanks Dr. Taka. You are on the same line as my urosergeon. I have the findings from a closed MRI that will be used to map the biopsy. Will you please read over the findings and compare them to the findings of the first MRI and share with me if there are any changes and if proceeding with the biopsy should be the next step in your opinion.If yes to the biopsy would putting it off for 2 weeks be inadvisible. This would be for personal reasons. Or should it be done right away. Also give me an approximate time for recovery from biopsy.

These are the last MRI finding: Contrast enhance RI images of the brain were obtained using a localization exam. No previous studies are available here for comparison.

There are two adjacent hyperdense ( and presumably enhancing) lesions present on the right side, located in the right insular region (2.1 cm and slightly inferior to this, also in the sub insular region, measuring 1 cm. This legion is slightly hyperdense on the T2-weighted images.

No other enhancing lesions are abnormalities are noted.
Correlation with previous imaging recommended.

IMPRESSION: Two adjacent insular intra-axial lesions are described bove

There is a change in the second MRI as it describes not one but two lesions this time which would go more with the diagnosis of a tumor. However it would have been good if the interpreter had at his/her disposal the images from the first MRI as well to make a better comparison side by side (the report states those images were not available).

I wouldn't wait 2 weeks for the biopsy. It is not urgent in the sense it can wait a few days, but 2 weeks are too risky. I do not know what techniques your neurosurgeon uses, but these days biopsies are done stereotactically (imaging techniques guide the procedure) and you can be back home the next day (at times even the same day) to attend to your personal issues. This way a more precise diagnosis and management plan according to biopsy results can be done, perhaps giving you time to close those personal issues in piece of mind.
