Question: Hello, I just spent about 20 minutes at a meeting at work sitting right next to someone who was speaking so loud it was almost as if they were shouting in a football stadium. A few minutes after they started this shouting, my ears started to hurt and feel "full." When the person stopped shouting, other people, speaking in more normal tones, did not sound as clear to me as before. It's been an hour since the meeting ended and my right ear still hurts and sounds are less clear than before. Have I suffered permanent damage to m
y hearing or will this heal on its own? Thank you very much for your help.

An acute exposure to loud sound can lead to discomfort but damage in your case is unlikely as the source was someone talking which is not that high in frequency.
You should take rest and relax for sometime. The feeling will subside. If still you feel that there is some discomfort you should get a pure tone audiometry. Any further management will depend on the reports.
Take paracetamol for pain.
Avoid XXXXXXX phone usage and ear phones usage and any exposure to loud sound.

Dear Dr. Aggarwal,

I forgot to mention that several years ago, I had a perforated tympanic membrane in my right ear, after flying with a bad cold. I had noticeable hearing loss after about a month, but recovered and have not noticed hearing loss since, nor had ear problems since. However, my right ear, after popping for a while as if I'd been on an airplane, is still aching and still feels full nearly 3 hours after exposure to 20 minutes of what was essentially shouting in a small room in an otherwise quiet environment (so that the onset of loud noise was sudden as well as very close by and then continuous).

Correction to my most recent message above: After my tympanic membrane perforation, I had hearing loss FOR about a month, not "after" about a month. Thank you again for your help.

Thanks for the information.
As you said the perforation has healed, so there is nothing to be worried about but if you have that popping sensation again and again, you should get it investigated for any nasal pathology or any other cause for eustachian tube blockage because it can further lead to ear problems.
My advice is you should now relax and rest for at least 2 days. If the symptoms still persist than get audiometry done. Once you feel better, get a x-ray paranasal sinuses done.
Take steam inhalation and chew a gum that will also help decongest the tube and relieve the symptoms.
