Suggest Treatment For Symptoms Of UTI

Question: My mother whose 78, has been diagnosed with UTI, this has started like 8 months back where the local doctor used to treat her with antibiotics and it would get better and then after a month or 3 weeks return again, the last time we had sent her XXXXXXX where they did extensive testing and said there is urine retention problem, for which they had a small surgery to increase the space so urine can pass freely, after that surgery its now like 3 months and the symptoms appeared again, last time she was treated with nitrofurantoin, for 10 days and then again was given for another 10 days in XXXXXXX but at the moment i dont know how to get a cure from this ESBL E.Coli.

Hello,

since she's having too many urinary tract infections, she has to take prophylaxis. Treatment comes first. The treatment regimen should be according to the results of the antibiogram. Treatment should be prescribed only if she has symptoms. Otherwise no treatment is necessary.

Regarding prophylaxis she can take either a low dose antibiotic (like low dose trimethoprim-sulfamethoxazole) or other substances. Cranberry extract is a very good example of such preventive measures. It has been shown to reduce lower urinary tract infections. It can't be used when there is already an infection but it can be used to prevent the next one.

A careful review of her drugs is necessary as well. Certain drugs may increase the risk for urinary tract infections (empagliflozin is an example). This is the least important aspect though.

Hi,

I've taken a look at your reports. It seems that you had a stenosis and an attempt to correct it was undertaken. I've seen that the current pathogen is sensitive to nitrofurantoin which is a good choice for oral treatment. So she to has to eradicate it and receive prophylaxis afterwards. In patients like her I usually suggest cranberry extract which contains proanthocyanidins (PACs). 36mg of PACs twice daily should be enough for prevention. If the cranberries are not effective to prevent the infections, then low dose trimethoprim-sulfamethoxazole can be tried, unless there is any contraindication for this drug.

You're welcome!

You should look at the OTC and supplements section for cranberry extract containing PACs (proanthocyanidins ). If you can't find it, you can look for TMP-SMZ (or TMP-SMX it's the same) (trimethoprim-sulfamethoxazole). This is not a brand name. They are the active substances. A well known brand name containing TMP-SMZ is Bactrim. I don't know which is the brand name in your country. You should ask your pharmacist for that.

Hi,

I don't know how many PACs each tablet contains but she'll need 36mg twice daily (72mg per day) to achieve the best results. You can check the package for the ingredients. Watch for PAC (proanthocyanidins) which is the ingredient of interest in her case. I searched in the web but I couldn't get more information about this package...

If the cranberries don't work as expected then she can proceed with septrin. Remember to ask her doctor first for any contraindications. Some patients should not use septrin. If she's OK to use it then a low dose (1 pill) every other day should be enough. That is one pill today, no pill tomorrow, one pill the day after tomorrow and so on.

If she still has more 50cc residual urine volume in the bladder after voiding then perhaps she has to consult the urologist once again.

The dosage for septrin should be 1 pill every other day.

I cannot tell you the dosage for this particular cranberry product because I don't know (and can't find) the PAC content. Perhaps there are instructions inside... I'll be glad to comment on any questions you may have regarding its instructions, if you could upload a copy.

Hi,

the last culture revealed a nitrofurantoin resistant pathogen so nitrofurantoin cannot be used. You can use septrin though, if there is no contraindication (hypersensitivity to the active ingredients mostly). Two tablets twice daily (four tablets in total daily) is the recommended dosage for 10-14 days. Make sure that the most recent creatinine levels are normal or near normal. If they're not please let me know, to calculate the right dosage.

After completing this course you can start the cranberries...
