Question: My wife has XXXXXXX diagnosed with stage 4 breast cancer that has move to sever vertebrae in her spine, Recently her feet have begging swelling and she has had spells of falling and being unconscious. she just had a dipolar ultra sound and the found her inferior vein cava push up and touch her aorta what she we ask her oncologist.

1. There is a need to know why your wife is having spells of falling and being unconscious. This might be discussed with the oncologist. It is possible that the cancer might have spread to the brain or her blood count is low and she is having severe weakness. You might ask the oncologist if she requires a brain CT or MRI scan.

2. The ultrasound Doppler report is to be studied in detail. It is important to know whether there are clots in the lower limb veins or inferior vena cava and if that is the reason for swelling of feet. This is usually painful swelling and this point might be discussed with the oncologist.

3. Giving your wife the right diet also needs to be discussed. It is possible that the oncologist might consider giving chemotherapy and radiation therapy. These might make the patient weak and it is important to keep your wife healthy and maintain her general health. She might be given daily 1800 kcal diet with proteins.

4. Falsodex, Xgeva and anastrozole are medicines recommended for breast cancer that has spread to other organs. Your wife also requires dietary supplements of vitamins and minerals and to keep her bones strong.

What would cause that vena cava to push up to her aorta. Couldd a tumor on her liver cause that?

It is important to know at which level the inferior vena cava is pushed towards the aorta.

The possibility of a liver tumor causing this is less likely. However other causes like a thrombus in the inferior vena cava or any bone destruction in the spine with soft tissue component is more likely to cause it.

It is difficult to say without reading the Doppler report.
