Brief Answer:
Needs detailed autoimmune workup and rheumatologist review

Detailed Answer:
Hi! Welcome to health care magic! Thanks for sharing your concerns with us. We will try to help you in best way possible.

Based on the history you have shared, the symptoms support some sort of systemic vasculitis and needs detailed evaluation by a rheumatologist for autoimmune diseases. However, to start with baselines labs like complete blood count, clotting profile to rule out any bleeding tendency (to assess symptoms like right cheek bone mark and to rule any bleeding in the joint which may cause severe knee and ankle pains). Other workup like ANA, ESR, CRP, RA factor and XXXXXXX as part of initial auto immune workup.

The right leg swelling should also be evaluated on blood workup like D-dimers and fibrinogen and ultrasound doppler of right leg, to rule out any clot in the swollen leg. The painful joints also need evaluation of Xrays of affected joints, Uric acid levels and calcium, phosphate level and vitamin D levels.

CPK and LDH may also be needed to rule out myopathy due to symptoms like loss of muscle mass.

Meanwhile taking OTC analgesics like acetaminophen can be safe for supportive management and capsaicin ointments on joints to decrease pains.

All.this workup is still basic initial workup with such symptoms as these can't be ignored neither can they be summarised under one specific diagnosis except for systemic vasculitis like SLE however other causes like infective diseases and malignancies should be considered afterwards. Such Patient needs indoor admission and detailed workup rather that on outdoor basis.

Hope this has answered your query, if you have any more questions, feel free to ask. Regards.
