Suggest Treatment For Sore Behind Lip After Performing Cunnilingus

Question: About a 4 weeks ago I had a short instance of giving cunnilingus to a Chinese prostitute in the U.S. About three weeks later I think I bit my lip (not sure) and developed a sore on the back of my lip that I thought looks like syphilis. The sore started to heal quickly and within six days it healed completly. I have attached two pictures. The first is when The sore first developed the second is 2-3 days later. Does this look like primary syphilis?

I have reviewed images. They both are same image. In image I can see a single sore on the inner aspect of lower lip.

An oral sore after a recent history of unprotected oral sex could be either one of the STDs like syphilis, herpes. Possibility of traumatic sore and aphthous ulcer/ canker sore also need to be considered.

I cannot be absolutely sure about the true nature of this sore just by having a look at it. A physical examination should provide better idea about the true nature of it. A syphilitic sore is painless, indurated i. e hard at base and clean based. There is also regional lymph node enlargement i. e cervical lymphadenopathy in case of an oral sore of syphilis. Primary chancre of syphilis if left untreated would resolve in 1-3 weeks.

In addition to physical examination I would suggest a test for syphilis i. e VDRL in order to be absolutely sure. This test first becomes positive usually a week after the appearance of chancre and stays positive thereafter.

I would suggest that you get tested for herpes as well. A blood test for herpes i. e ELISA for type 1 and type 2 Hsv can be taken at 4 weeks from sexual encounter.

Given that the day before the sore appeared I bit my lip in that same place would that be a consideration? Is the picture similar to a sore that would come from trauma?

This being a traumatic sore is a possibility, as I already mentioned in my previous reply, but considering that you had a recent unprotected sexual contact, therefore the other possibility i.e STDs also need to be reliably ruled out.

I took an rpr 9 days after sore appeared, would you say it will be accurate?

Yes, an rpr test is reliable if it is done after a week of chancre

Yes, an rpr/ vdrl test becomes positive a week after primary chancre. So this test at 9 days would be reliable.
