Brief Answer:
Stress test is needed for the correct diagnosis

Detailed Answer:
Dear Sir,

I would advise you to undergo exercise stress test.
It will help to find out if you have coronary artery disease (blockages in heart arteries) .
Diabetic patients have more risks to develop coronary artery disease, which can cause symptoms you describe. And if disease is diagnosed in early stages and treatment started, then heart attack can be prevented.

Hope I could help you

Wishing you good health
