Question: My heart bpm has been all over the place. I am 68", 260lbs, and 72 yrs old. Bpm sleeping is between mid-40s to into 70s. Awake resting between 60's and mid 80s. Walking around or doing some minor work, generates 105 to 154, shortness of breath, and some minor chest discomfort. Have been diagnosed with a minor enlarged heart (no meds - weight reduction), and acute bronchitis and early stage enfazema.

Heart rate more then 100 is tachycardia and less then 60 is called bradycardia.Both the conditions can be dangerous hence a thorough investigation of cardiac system should be done to rule out the cause.
I would suggest you to go for two baseline tests of cardiology like
24 hour monitor - It will record your heart rate and nature of beats for 24 hours,Hence we could find out the highest and lowest heart rate you have in a day and nature of heart beat at that time.
2d echo -To see if there is any further cardiomegaly.As your heart is already enlarged.
