Question: My father had vertigo last month. Post which we got the complete health check up done. In health check up we came to know that his heart is pumping at 50-55℅ and b12 is below 100, also kidney has deteriorated. RBC, WBC, PSV and platelets count is around 85k. Doc gave him 10 b12 injections and medicine for kidney. Few days back as the RBC, WBC and platelet count did not improve doc suggested to visit hematologist. we also ruled out Dengue by test. Hematologist did initial tests for PT, a PTT, S.Ferritin, HIV. Now hematologist is asking to get Bone marrow, cytogenetics and FISH for MDS. can u pls suggest shud I go for these tests?? and any alternative of what is the problem?

Brief Answer:
Yes this plan seems reasonable

Detailed Answer:
Hi
Thanks for your query.

B12 deficiency can cause low RBC, WBC and platelets. But that should improve with b12 supplements. It may take more time however.

If still not improving then bone marrow will be required. It will confirm changes of b12 deficiency or refute it. In that case it will show what can be the other cause. One of them is MDS.

So I feel that unless the blood parameters are improving soon, a bone marrow with cytogenetics is in order.

Hope this helps.
Regards
