Suggest Treatment For Severe Stiffness In Back And Abdominal Muscles

Question: I am suffering from ankylosing spondylitis . I am having back pain and severe stiffness in back and abdominal muscles simultaneously after sleeping. I could not stand for half hour after sleeping positions. As per CT Scan reports , (I) Vertebral alignment from L1 to L5 level appears normal. No evidence of dislocation /fracture seen. (ii) Syndesmohytes are seen at all lumber and lower dorsal levels. (iii) Fusion of facet joint seen at lumber and lower dorsal level with additional changes of degeneration at places.(iv) Ossification of interspinious ligament is seen (v) Fusion of visualized sacroiliac joint is seen in on both sides (vi) Pre and para vertebral soft tissues are normal. Please note that THR both hips done one year before. Please guide us whether MRI of spine could be done after THR. Currently I am taking Sulphasalazine 500 mg 2 times, Indomethacin , Shelcol OS regularly.

Please suggest additional medicine for this pain and stiffness.

After metallic implants in the body like THR an MRI scan cannot be done for evaluation of the spine. The CT scan is suggestive of spondylottic changes in the spine which is causing the pain and stiffness. In addition to the medicines that you are already taking you can add an oral muscle relaxant like thiocolchicoside after consulting your orthopedician. This will help in relieving the muscle spasms. Increasing your daily fluid intake will also help.

Strengthening the muscles that support the spine helps keep the spine in its proper position and reduces risk of sprains, strains and lower back pain. Maintain fitness with exercises that include back stretching and strengthening exercises under guidance of a physiotherapist. This will help in retarding the further progression of the disease.
