I am presenting with significant foot...
I am presenting with significant foot pain.

I have biomechanical issues (over-pronate, bunions, & in the past metatarsalgia). I wear 3/4 orthotics. I have never had rear foot pain.

Since the start of September I have had significant rear/mid foot pain when walking/load bearing but initiallypresented on the inside arch of both feet (not the sole) and also just below the inside of the ankle. With some rest, ice and very supportive walking boots I thought it had stopped and I walked freely again mostly on the beach. When I walked more on hard pavements it started again and is slightly different and considerably worse this time.

It gets progressively worse as I load bear and the day progresses, it is a aching at rest now and can sometimes become extremely painful for no reason. It wakes me at night. The pain is still felt in the same places as before, particularly when driving but is also at the back on my foot where my heel and sole meet. it is sore lying on my back with that area of my feet touching the mattress. It also aches to put the inside of my foot on the mattress. It was mostly my left but my right is becoming increasingly worse. I tried icing it again last night but the heel/sole area where the meet was excruciatingly painful until I removed the ice. I have today swelling just under the bone on inside of ankle. I cannot tolerate tighter shoes around the back of my foot or the pain is unbearable. I pressed the painful swollen area just under the ankle bone on inside of foot and it was very painful. I cannot take NSAIDS.

I am uk based. I am waiting on referrals to orthopedic, physio and podiatry (5-18 months wait). I am unable to begin to treat my problem with any great effect as no one knows what it is. I understand without seeing my foot you cannot give a definitive answer. I am in the caring profession and work on my feet most of the day.

Also, My feet feel stiff. I am getting some arch pain on inside edge of foot like something cramping or stretching even when at rest. This sometimes is in my sole too. Pain where heel/sole meet is different kind of pain. My other foot continues to worsen also, with limping. I have snapping sounds regularly in both feet when moving them mostly on the insides of feet not in the sole.

What could it be?
What diagnosis do you think is most likely? I understand you cannot say definitively.
How is this best treated until I see specialists?
What would be the ideal treatment?
Do I ice if the pain is intolerable?

The pain that you have described is likely to be due to metatarsalgia with plantar fasciitis. All this happens due to inflammation in the different areas of plantar fascia and in the metatarsal joints.

For relief, the first thing is avoid putting stress on your feet. Avoid walking on uneven surfaces, avoid standing for too long and use a soft sole shoes.

The inflammation can subside with the help of anti inflammatory pain killers or oral steroids. Local steroid injections will also help in case the pain is persistent. To get a temporary relief, it is best to do a hot compress in the area of pain and apply a local analgesic (eg. Diclofenac) gel.
